Please arrive 15 minutes prior to your scheduled appointment time to allow time for check-in and paperwork processing. We kindly ask that all required digital forms linked below be completed in advance of your appointment. Completing your paperwork ahead of time helps ensure a smooth and efficient flow for everyone throughout the day.
Appointment Requirements
All services REQUIRE an appointment.
Basic Cardiac: Cardiac auscultation performed by a veterinarian for OFA cardiac clearance. If an echocardiogram is recommended or required by breed, this will be discussed at the time of the exam. OFA submission fees are not included. You as the owner are responsible for submitting the paperwork to OFA.
OFA Hip, Elbow & Spine X-Rays: Radiographs performed for official OFA evaluation of hips, elbows, and/or spine. Dogs must be at least 24 months old for final certification (preliminary evaluations are accepted for younger dogs at 12 months.) All dogs must be fasted prior to this appointment in case sedation is required. Sedation is NOT included in the listed pricing and is weight-based. We are happy to provide an estimate prior to your visit. Digital copies of radiographs will be emailed to owners the day of the clinic. OFA submission fees are not included. We will submit your OFA form & X-Rays on your behalf. Results can take anywhere from 2-4 weeks after submission.
Patellar Luxation (OFA Patella Certification): Physical orthopedic examination performed by a veterinarian to evaluate for patellar luxation (kneecap stability). This is a hands-on exam and does not typically require sedation. OFA submission fees are not included. You as the owner are responsible for submitting the paperwork to OFA.
OFA Thyroid (OFA Thyroid Certification):Blood sample collected and submitted to an approved laboratory (Michigan State) for OFA thyroid evaluation. OFA submission fees are not included. We will submit your OFA form & X-Rays on your behalf. Results can take anywhere from 2-4 weeks after submission.
Brucellosis: Blood test performed to screen for Brucella canis, a contagious bacterial infection that can impact fertility and breeding programs. This test is strongly recommended prior to breeding. Results are typically available within a few days depending on laboratory processing time. This is NOT an OFA Test.
SpermVisions CASA Sperm Analysis: Advanced computerized semen analysis using SpermVision® CASA (Computer Assisted Semen Analysis) technology. This evaluation provides objective measurements of sperm concentration, total sperm count, motility, morphology, and progressive movement using digital imaging software. A full report will be provided once assesment is completed. If your dog has not been collected before, we do recommend you bringing a female in heat as a teaser. We do NOT provide this service. This is NOT an OFA Test. A fee will be charged for the service regardless of whether a semen sample is successfully obtained.
Forms & Registration
You will be asked to complete the examining veterinarians’ forms for cardiac and eye exams. Please bring your dog(s)’ AKC registration information and allow time to complete forms before your appointment time.

By submitting your appointment request, your selected credit or debit card will be charged.
No refunds for no-shows.
If you need to cancel, you must contact Shaina at repro@damcvets.com no later than April 10th.
Appointment Scheduling
Once we receive your submission, appointments will be scheduled in the order that requests are received. We ask that all participants remain flexible throughout the day, as appointment times will be assigned and confirmed chronologically based on the submission queue and the type of tests selected. Final schedule will be emailed about 2 weeks prior to our OFA Clinic date.
