Bus Transport Registration Form
Please fill out the form below to register for BUS TRANSPORT for the church camp.
Pls note that this is only the registration for the BUS TRANSPORT. You will still need to register for the church camp separately: https://form.jotform.com/mtcarmelsg/mount-carmel-church-camp-2026
Instructions:
1. Every passenger travelling on the bus must be registered.
2. We will aim to place passengers who register using the same form on the same bus.
3. However, bus allocation is primarily based on hotel location. Each bus will serve only one hotel, so passengers staying at different hotels will not be assigned to the same bus, even if they register on the same form.
4. Cut-off date for registration for bus transport is 1 May 2026.
5. Transport cost per passenger: $$35/one-way/ passenger; $49/two-way/passenger.
6. Make payment transfers (Impt: Pls include your contact number, name, and “BUS” in the payment reference, and email a screenshot of your payment to churchcamp@carmel.sg. Eg. ‘91234564 Alan Tan BUS’)
