Health Form Policy
If your child is new to Westminster, a physical examination completed by your child's medical provider is required. The form can be completed up to one year before starting school and can be found at this link if you want to get a head start on this requirement!

If your child is returning to Westminster, your child’s health form is already on file and a new one is not required. However, you must provide an updated immunization record whenever your child receives more immunizations.

More information about student forms will be sent over the summer!
