**INFLATABLES, MECHANICAL RIDES & EVENT ACTIVITIES
RELEASE OF LIABILITY, WAIVER OF CLAIMS,
EXPRESS ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT**

Event Provider: Chrisally Events and Party Rentals (“Chrisally Events”)
School/Host: Simpson Middle School

This Release of Liability, Waiver of Claims, Express Assumption of Risk, and Indemnity Agreement (“Agreement”) is executed by the undersigned Parent or Legal Guardian on behalf of themselves and the minor participant listed below.

ACTIVITIES COVERED
This Agreement applies to any and all activities, attractions, equipment, and entertainment provided by Chrisally Events and Party Rentals at Simpson Middle School, including but not limited to:

Toxic Meltdown
Mechanical Bull
65’ Obstacle Course
The Vault Interactive Challenge
Shooting Stars Basketball Inflatable Game
Dunk Tank
Interactive Challenges
Any other inflatable, mechanical ride, game, amusement device, concession-related activity, or entertainment provided before, during, or after the event

ACKNOWLEDGMENT OF RISK
I understand and acknowledge that participation in inflatable attractions, mechanical rides, interactive games, and event activities involves inherent risks, including but not limited to:

Slips, trips, and falls
Collisions with other participants or equipment
Mechanical movement, sudden stops, or imbalance
Physical exertion, impact injuries, or overexertion
Equipment failure or misuse
Weather-related hazards
Serious bodily injury, paralysis, or death
I voluntarily choose to allow participation with full knowledge of these risks.

1. RELEASE, WAIVER & COVENANT NOT TO SUE
In consideration of my child being permitted to participate in the Activities, I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE:

Chrisally Events and Party Rentals
Chrisally Group LLC
Simpson Middle School
Their officers, employees, agents, volunteers, contractors, representatives, and affiliates
(collectively referred to as “RELEASEES”)

from any and all liability, claims, demands, actions, or causes of action, including those resulting in personal injury, illness, property damage, or death, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, arising out of or related to participation in the Activities or presence on the premises.

2. EXPRESS ASSUMPTION OF RISK
I knowingly and voluntarily ASSUME FULL RESPONSIBILITY FOR ALL RISKS, known or unknown, foreseeable or unforeseeable, associated with participation in the Activities, INCLUDING RISKS ARISING FROM THE NEGLIGENCE OF THE RELEASEES.

3. INDEMNIFICATION & HOLD HARMLESS
I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS the RELEASEES from any and all claims, damages, losses, liabilities, costs, or expenses (including attorney’s fees and court costs) arising from:

My child’s participation in the Activities
My own actions or omissions
Claims brought by or on behalf of the minor participant

4. MEDICAL CONSENT
If the participant is under 18 years of age, I hereby:

Consent to my child’s participation
Authorize Chrisally Events and Simpson Middle School to obtain reasonable and necessary emergency medical treatment
Agree to be financially responsible for any medical expenses incurred

5. INSURANCE ACKNOWLEDGMENT
I acknowledge that I am responsible for reviewing my personal health and accident insurance coverage and understand that the RELEASEES do not provide insurance coverage for participants.

6. BINDING EFFECT & GOVERNING LAW
This Agreement shall be binding upon me, my family members, heirs, assigns, and personal representatives.
This Agreement shall be governed by and interpreted according to the laws of the State of Georgia.

7. ACKNOWLEDGMENT OF UNDERSTANDING
I certify that:

I have read this Agreement in its entirety
I fully understand its terms
I am signing it voluntarily and knowingly
No oral representations or inducements have been made
I am the legal parent or guardian of the minor participant
