About Molly Smiles Foundation, Inc. (“MSF”) Grants
1. The application for a grant is a two-stage process. Once you submit your initial screening application, the MSF board of directors (the “Board”) will decide whether to invite the eligible candidates to submit additional supporting documentation.
2. Grantees are selected by majority vote of the Board. In order to help as many families as possible, MSF may award up to a $1,000 maximum grant per recipient family (with rare exceptions made at the Board’s discretion).
3. Children and their primary caregiver must reside in California to be eligible for a grant. Priority will be given to applicants who reside in the San Francisco Bay Area who are experiencing financial hardship.
4. Grant applicants are required to provide the total amount of their grant request to MSF during the application process. Upon approval, MSF will issue a direct payment to the vendor, service provider, or other relevant entity for the approved item, service, copayment, or related expense on behalf of the grant recipient.
5. All candidates that comply with MSF’s grant requirements are eligible to apply annually.
6. Grantee documentation submitted to MSF in connection with a grant application will be kept secure and only shared with MSF team members as part of the grant selection process.
7. Noncompliance with the terms of the grant, or false statements made in connection with the application or management of grant funds shall be cause for termination of the grant, and MSF may require reimbursement of any payment made.
8. MSF may use any photographs, stories, video and other independently gathered information about the grantee as part of its efforts to raise awareness and expand our mission, including as part of fundraising activities and markets programs.
9. Submission of a grant application initial screening constitutes your agreement to these terms.
