Registration Form Information
• Please complete the form for the student who will be participating in the class.
• Be sure to use the correct email address and double-check spelling.
• The blended learning portion will be sent to the email provided.
• Once the form is submitted, an invoice from Genesis will be sent within 24–72 business hours.
• Your spot is NOT secured until payment has been processed.

Payment & Course Requirements
• Payment must be processed and confirmed at least one week before the course begins.
• The online portion of the course must be completed before the first in-person class.
• The online course link will be sent one week before class, once payment is received in full.
• After payment is processed, you will receive a confirmation via text or email.
• This confirmation must be shown on the first day of class.

Important Notes
• There is a minimum of 5 students required for the class to run.
• If the minimum is not met, you will be moved to the next available class.

Weekend Classes Information

Weekend class options are held Friday from 5:00 PM–9:00 PM, Saturday from 8:00 AM–6:00 PM, and Sunday from 8:00 AM–6:00 PM.There is also a Thursday/Friday/Saturday class option held Thursday from 5:00 PM–9:00 PM, Friday from 5:00 PM–9:00 PM, and Saturday from 8:00 AM–6:00 PM.Classes will be held at either Genesis or Cady Way Swimming Pool, depending on instructor scheduling.Please arrive 10 minutes before class to get checked in. An instructor will be waiting at the front desk.Please make sure you are selecting the correct class option when registering.

Swim Test & Deposit Option (Required)

Pay a $15 drop-in fee to come in one week before the class to complete the swim test (if you choose to test out based on proficiency). This $15 will be applied as a deposit and deducted from the total class cost. The swim test must be completed at least one week prior to the course due to enrollment requirements. By selecting this option, I confirm that I have read and understand the policy above.Forfeit the $15.00 deposit, pay the full amount for the class, and be required to take the swim test on the first day of class. If you do not pass the swim test, the class payment is non-refundable as the swim test is a prerequisite of the course and is not negotiable to continue in the course without passing the prerequisite.
