Pizza Order Form for October 30th

Let us take care of lunch on Friday, October 30th. Pre-Order pizza and/or cookies for your student below. A very limited number of slices and cookies may be available to purchase at lunch (cash only) while supplies last.

Pizza is $5.00 for 2 slices and students can purchase 2, 4, 6, or 8 slices of the same topping (note: 8 slices is a whole pizza). Gluten Free pizzas are available for $15 for a whole small pizza (GF is not available by the slice).

Cookies are $1 each and can be ordered as an add on to pizza or on their own for those who are not interested in pizza. Gluten Free cookies are available.

Checks should be made payable to Downing Band Boosters. Please put your cash or check in an envelope marked with your child's name and "Pizza Lunch".

All cash/check payments should be dropped off in the main office at DMS or in the lock box located in the band hall.
