    Please complete this form to redeem your certificate. All redemptions are subject to the stated terms and conditions. Certificate Holders are responsible for any additional costs, including upgrades, taxes, and fees not covered by the prize. Failure to submit accurate information or meet redemption deadlines may result in forfeiture of the certificate.

  • Certificate Details

    As of May 7th, 2025 you MUST BE Real ID compliant to travel Domestically or a valid passport. Weekend Getaways, Flight/Hotel Combo, Flight Only, and Hotel Only can accomodate up to 2 people. Family Vacations can accomodate up to 4 people per Certificate. 10K Famiy Vacations are not permitted to be stacked under any circumstances.

  • Travel Information

    Please provide details about your preferred travel plans. All travel arrangements are subject to availability, blackout dates, and eligibility requirements. Requested travel dates must be at least 4 to 6 weeks from the date this form is submitted. Travel requests with less than 4 weeks' notice may not be accommodated.

  • Flight Information

    Flights are only booked in Economy Class and are subject to availability, airline policies, and blackout dates. Travelers are responsible for any additional costs, including baggage fees, seat upgrades, or itinerary changes.

  • Passport Information

    Required for international travel. Please ensure your passport is valid for at least 6 months beyond your travel dates. Important Notice: Travelers are responsible for obtaining their own passports and visas. Failure to secure the necessary travel documents may result in forfeiture of the certificate.

  • Hotel Information

    Hotel accommodations are subject to availability and may include restrictions such as blackout dates and room type limitations. Upgrades, additional nights, or special requests may be available at an additional cost.Important Notice: Hotel reservations are based on standard accommodations unless otherwise specified. Any additional charges, such as resort fees, incidentals, or room upgrades, are the responsibility of the traveler.

  • Redemption Period Acknowledgment

  • By submitting this form, I acknowledge that I have reviewed and understand the redemption period associated with my travel certificate. The redemption period was identified at the time of booking the certificate, and I am responsible for adhering to the specified timeframe for travel arrangements.

    Key Terms & Acknowledgments:

    ✨ Holiday Processing Notice: Due to the holiday season and increased volume, standard processing time is 6–8 weeks. Reimbursements over $2,000 may take up to 10 weeks to process. We appreciate your patience and commitment to our community experience!


    -Certificate Expiration: The certificate must be redeemed within the designated redemption period. Failure to do so may result in forfeiture of the prize, with no extensions or substitutions allowed.

     

    - 90 day Gap: After use of a certificate there will be a 90 day gap before you can redeem another. Please note that this applies to previously purchased and newly purchased certificates. Does NOT include - Spa, Dinner or small flight certificates


    -Booking Deadline: All travel arrangements must be confirmed within the required timeframe as communicated at the time of booking the certificate. Late submissions or failure to book within this period may render the certificate void.


    -Advance Notice Requirement: Travel requests must be made at least six weeks in advance of the preferred travel dates. No last-minute bookings will be accepted. Requests submitted with less than six weeks’ notice may not be accommodated.


    -Availability & Restrictions: Travel is subject to availability, blackout dates, and booking restrictions. If preferred travel dates or destinations are unavailable, alternate options may be provided.


    -Non-Transferability: The certificate is non-transferable and may only be redeemed by the original recipient as recorded at the time of issuance.


    -Certificate Value & Additional Costs: I understand that my certificate covers up to the amount specified on my certificate. If my selected flight or hotel exceeds this amount, I am responsible for covering the difference. Any additional costs, including taxes, fees, upgrades, or personal expenses, are my sole responsibility.


    -Required Documentation: I understand that I must provide valid identification, passports, visas (if applicable), and any other necessary travel documents. Failure to provide these may result in forfeiture of the certificate.

    By signing below, I confirm that I have read, understand, and agree to the redemption period and terms outlined above. I acknowledge that failure to comply with these terms may result in the forfeiture of my travel certificate.
