This onboarding form must be completed in full before dispatch services begin.
By continuing, you acknowledge that you will be entering into a legally binding
Carrier–Dispatcher Agreement with electronic signature.

 

Dispatcher: Path of Excellence Logistics

1. Services
Dispatcher agrees to provide freight dispatch services including sourcing and
negotiating loads. No guarantee of load volume or revenue is made.

2. Independent Contractor
Dispatcher is an independent contractor and not an employee or agent of Carrier.

3. Dispatch Fee
Carrier agrees to pay a dispatch fee equal to 10% of the gross rate for each load.

4. Payment Terms
Payment is due within 3 calendar days of Carrier receiving payment from the broker.
Late payments may result in suspension of services.

5. Carrier Responsibilities
Carrier is responsible for maintaining active authority, insurance, and FMCSA
compliance.

6. Confidentiality
All broker contacts, rates, and business information are confidential.

7. Term & Termination
This Agreement remains in effect until terminated by either party with 30 days
written notice. Outstanding fees remain due.

8. Limitation of Liability
Dispatcher is not liable for broker non-payment, cancellations, or accessorials.

9. Governing Law
This Agreement shall be governed by the laws of the State of North Carolina.

10. Electronic Signatures
Electronic signatures are legally binding.

This electronic signature is legally binding and equivalent to a handwritten signature.
