1. Acknowledgment of Risks
I, the undersigned parent or legal guardian, understand that participation in birthday parties, special events, tennis, pickleball, games, inflatables, and other physical or recreational activities involves inherent risks. These risks may include, but are not limited to, falls, collisions, physical contact, equipment-related injuries, weather exposure, and other potential hazards.
I voluntarily assume all such risks on behalf of my child and confirm that my child is physically and medically able to participate.
2. Release of Liability
In consideration of my child being permitted to participate in activities at Mikas Tennis Academy, I hereby release, waive, and discharge:
Mikas Tennis Academy, its owners, directors, officers, employees, coaches, contractors, volunteers, and affiliates
from any and all claims, demands, losses, causes of action, or liabilities of any kind, whether known or unknown, arising out of or related to my child’s participation, including but not limited to injury, illness, property damage, or death, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Indemnification
I agree to indemnify and hold harmless Mikas Tennis Academy from any claims, lawsuits, damages, costs, or expenses arising out of my child’s actions, participation, or presence at the facility.
4. Medical Authorization
In the event of injury or illness, I authorize Mikas Tennis Academy staff to seek emergency medical treatment for my child if necessary. I understand that I am solely responsible for any medical expenses incurred.
5. General Terms
I certify that the information provided is true and accurate.
I agree that my child will follow all facility rules and staff instructions.
I understand that failure to follow rules may result in removal from the event without refund.
