Waiver and Liability Release: I, the parent/guardian fully understand that Gibson Athletics Center LLC hereinafter referred to as GAC, staff members are not physicians or medical practitioners of any kind. With the above in mind, I hereby release the GAC staff to render first aid to my child or children in the event of injury or illness, and if deemed necessary by the GAC staff to call our doctor and to seek medical help, including transportation by a GAC staff member or its representatives to a health care facility or hospital, or the calling of an ambulance for said child should the GAC staff deem this to be necessary.
Students may suffer injuries, possibly minor, serious, or catastrophic in nature. Gymnastics, cheerleading, trampoline and other activities under the GAC programs can be dangerous and can lead to injury. Parents should make their children aware of the possibility of injury and encourage their children to follow all the safety rules and the coaches’ instructions. GAC, its coaches, instructors and other staff members, will not accept responsibility for the injuries sustained by any student during the course of any of the GAC programs, or open gyms or in the case of any exhibition, competition, or clinic in which he or she may participate while traveling to or from the event.
With the above in mind and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the programs offered by GAC. I, my executors, or other representatives, waive and release all rights and claims for damages that I or my child may have against GAC and/or its representatives whether paid or volunteer. I also affirm that I now have and will continue to provide proper hospitalization health, and accident insurance coverage which I consider adequate for both my child’s protection and my own protection. I also understand that it is the parents’ responsibility to warn the child about the dangers of gymnastics, open gyms, birthday parties, field trips or any other GAC program and injury. The parent should warn the child according to what the parent feels is appropriate. GAC will only warn the child through “Safety Messages” and our teaching style and progressions.
PHOTO & VIDEO RELEASE: I grant consent for my/minor’s picture to be taken or to be filmed while participating in activities at GAC. I authorize Gibson Athletics Center to use and publish images, photographs, pictures, portraits, and audio, video and/or film footage of me/minor in all forms of media and in all manner for publication including, but not limited to, advertising and marketing campaigns, press releases, periodicals, and website use. I hereby waive any right I may have to review, inspect, edit or approve such publication and I release Gibson Athletics Center from any claims I may have against it for use of such images, photographs, pictures, portraits, and audio, video and/or film footage of me.
