This information allows us to accommodate any specific health needs, allergies, or medical conditions your child may have, ensuring that we provide a safe and supportive environment for everyone. If certain health details are not applicable to your child, please indicate this by writing "N/A" in the relevant sections. This will help us understand that there are no specific health concerns to consider for your child's participation in our program. For families enrolling more than one child in our program, it's important to provide health information for each child individually. Please specify the name of each child alongside their respective health details to ensure we can accurately accommodate their unique needs and conditions.

By signing below and clicking the registration button, I acknowledge that I am 18 years of age or older and the legal guardian of the participant named in this form. I understand that Hidden Bee Learning follows a school-year model and that I am enrolling my child for the full school year. I confirm that I have read, understood, and agree to all the guidelines and policies outlined in the link provided.

Participation in the program indicates acceptance of these terms, including but not limited to:

Program Guidelines: I have thoroughly reviewed all policies, including payment requirements, attendance expectations, and any other program-specific rules. I understand that enrollment is a full-school-year commitment and that the notice, withdrawal, and payment terms are outlined on the Tuition and Enrollment page.

Participation Disclaimer: I understand that Hidden Bee Farm is not liable for any injuries or incidents that may occur during program activities. I acknowledge that adequate supervision and safety measures will be in place at all times.

Photo Release: I grant Hidden Bee Farm permission to use photos or videos of my child(ren) for educational, promotional, or marketing purposes. Additionally, in the event of a medical emergency, I give full consent and authorization for any necessary medical treatment to be administered to my child(ren), including evaluation, diagnosis, treatment, and transportation to a medical facility. I understand that all reasonable efforts will be made to contact me, but if I cannot be reached, I authorize the attending medical personnel to provide the care deemed necessary for the health and safety of my child(ren).

Please select only the days you are certain your child will attend. This helps us maintain accurate class sizes, ensure proper staffing, and prepare materials and activities for each session.

Selecting days your child is not expected to attend may take spots away from other families and disrupt planning for both the program and staff.

Little Explorers age 3-5, children must be age 3 at the time of enrollment in additional to potty trained.

Extended Care is a new option at Hidden Bee Learning. It is offered on a daily basis from 1:30-2:30(for both Little explorers and Pathfinders), it is optional therefor a seperate charge that will be added to your scheduled tuition invoice. Extended care is an entire school year commitment and will need a minimum number of students enrolled to be able to staff accordingly.
