Consent, Release & Waiver of Liability (Minor Participant)

Waiver Agreement for Participants Under 18 Years Old
1) Assumption of Inherent Risks
I understand participation in youth sports and related activities (practices, games, training, travel to/from events, and use of facilities/equipment) involves inherent risks, including but not limited to collisions, falls, sprains/strains, fractures, concussion/brain injury, cardiac events, dehydration/heat illness, and exposure to communicable diseases. I voluntarily assume all such risks on behalf of my child.
2) Release & Waiver of Liability (Including Ordinary Negligence)
In consideration for my child being permitted to participate, I, for myself and for the minor participant, hereby release, waive, and discharge Committed Sports, LLC, its owners, directors, officers, employees, contractors, volunteers, agents, sponsors, partner schools, and facility providers (“Released Parties”) from any and all claims, demands, causes of action, damages, losses, or liability arising out of or related to my child’s participation, including claims arising from the ordinary negligence of the Released Parties, to the fullest extent permitted by law.
This waiver does not release claims of gross negligence, willful/wanton misconduct, or where otherwise prohibited by law.
3) Indemnification / Hold Harmless
I agree to defend, indemnify, and hold harmless the Released Parties from any claims, damages, costs, or attorney’s fees arising out of my child’s participation, my child’s conduct, or my failure to follow program policies, except to the extent caused by the Released Parties’ gross negligence or willful misconduct.
4) Medical Authorization & Emergency Care
If my child becomes ill or injured, I authorize the Released Parties to seek and obtain emergency medical evaluation and treatment as deemed necessary. I accept financial responsibility for all medical care. I confirm my child is physically able to participate and I have disclosed relevant medical conditions/allergies on the registration form.
5) Concussion, Removal-from-Play & Return-to-Play
I understand and agree Committed Sports follows a remove-from-play policy for any suspected concussion. My child will not return to activity the same day and must have written clearance from a licensed health-care provider following a graded return-to-play progression. I acknowledge I have reviewed and accept the organization’s concussion protocol.
6) Safety, Conduct & Compliance
We will follow all posted and written rules, staff instructions, codes of conduct, and facility policies, including abuse-prevention and reporting policies, background-check rules for volunteers, equipment use, and no one-on-one/private communication boundaries between staff and minors outside approved channels.
7) Communicable Diseases
I acknowledge the contagious nature of illnesses (e.g., COVID-19, influenza, RSV) and voluntarily assume the risk that my child may be exposed to or infected. We agree to follow health/sanitation requirements (e.g., staying home when sick).
8) Transportation
If team/mentors arrange travel or carpools, I understand transport may be provided by independent contractors, parents, or third parties. I consent to reasonable team travel arrangements I select/approve and release the Released Parties as above for ordinary negligence related to transportation.
9) Media Consent

If you selected “Yes”:
You give Committed Sports permission to photograph, film, or record your child during program activities and to use their image/likeness, first name or first initial, and athletic achievements for marketing, highlights, website features, and social media.
If you selected “No”:
Committed Sports will honor your preference and make reasonable efforts to keep your child out of photos and videos. You understand that during group activities or public events, complete exclusion from all backgrounds may not always be possible, but the organization will avoid intentional media use featuring your child.
10) No Guarantee / Refunds
I understand playing time, team placement, or specific outcomes are not guaranteed. Fees are generally non-refundable after program start, except as outlined in the parent handbook.
11) Insurance & Subrogation
I acknowledge Committed Sports carries general liability coverage; this is not health insurance. I agree to use my personal insurance first for medical expenses and waive subrogation against the Released Parties to the extent permitted by law.
12) Governing Law, Venue & Severability
This agreement is governed by the laws of the State of Texas. Venue for any dispute shall lie in Bexar County, Texas. If any provision is held unenforceable, the remainder shall continue in full force.
13) Acknowledgment
I have read and understand this Consent, Release & Waiver. I am the parent/legal guardian of the minor named above, have authority to sign, and I sign voluntarily. I understand I am giving up legal rights on behalf of myself and the minor participant to the extent permitted by law.
