The Grand County Library District ("District") is offering an event where food will be served ("Event") and in which that food was not produced in a professional kitchen. This liability waiver ("Waiver) applies both on my behalf and on behalf of my minor children ("Participants"). I am the parent or legal guardian of the minor child(ren) and have the authority to enter into this Waiver on their behalf. Please read and sign below.

I understand participation is voluntary. By participating I assume any and all risks associated with the Event, including but not limited to personal injury, illness, damage to personal property, improper food preparation, risks associated with foodborne illnesses, contamination, food allergens including but not limited to nuts, dairy and gluten, and other potential health and safety hazards.

In the event of an emergency, I authorize the District staff to seek appropriate medical care. I understand that I am responsible for any costs associated with such medical care.

I understand that the District does not inspect or verify the quality, preparation, or ingredients of any food at the Event.

I understand that the Library has taken reasonable precautions to minimize these risks but cannot eliminate them entirely, and that I enter into this agreement and participate with the full understanding of all risks, known and unknown, including those arising from the actions, omissions, or negligence of the District or other participants.

I understand that by attending this Event, I hereby release, waive, discharge, indemnify, and hold harmless the Library, its Board of Trustees, employees, volunteers, agents, and affiliates (the "Released Parties") from any liability for any claims, demands, actions or causes of action arising out of or related to any injury, damage, or loss that I may suffer as a result of participating in the Event, including but not limited to third party claims and claims arising from the negligence of the Released Parties.

By signing this agreement, I affirm that I am fully aware of the risks involved with participating in the Event, and I am assuming all liability and responsibility for voluntary participation in the Event.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL AND ON BEHALF OF MY CHILD(REN).
