Our organization receives funding to directly support Wounded Warriors. While we define this very loosely, we would like you to provide information about your service history. Briefly describe your deployment history with the year(s), location(s) and unit(s) served. We are especially interested in knowing if you are a recipient of the Purple Heart and if you were injured in combat or while on active duty. Please do not share protected medical information and only share what you are comfortable telling us. We will not publish any information you provide and will only share information with charitable organizations to determine grant eligibility. If you do not wish to provide us with any information here, your eligibility to participate in our program will not be affected. Thank you for any information you choose to provide.
