  • This Contract outlines the expectations, responsibilities, and procedures governing the supervisory relationship. It is intended to protect the welfare of clients, support the professional development of the trainee, and clarify the ethical, legal, and professional obligations of both parties. This document may be revised as supervision progresses and will be reviewed periodically. If necessary, an addendum may be issued and will require review and re-signature by all parties.

  • I. PHILOSOPHY AND PURPOSE OF SUPERVISION

  • Supervisor Background. Shanae Farquharson is a licensed Marriage and Family Therapist in the state(s) of PA, DE, NJ, TX, NV, AR, OR, GA and an AAMFT Approved Supervisor Candidate. Shanae Farquharson, LMFT has been practicing clinically since 2015 and has provided supervision to pre-licensed clinicians since 2026. Shanae Farquharson has extensive experience working with clients experiencing anxiety, depression, stress, and family or marital difficulties.

    The supervisor’s clinical and supervisory approach is grounded in a systemic framework . Supervision is conducted using a developmental and competency-based model that emphasizes clinical skill development, ethical decision-making, cultural humility, and the intentional use of self in therapy.

  • Purpose of Supervision. The purpose of supervision is to:
    • Promote ethical, competent, and effective clinical practice
    • Support the trainee's professional identity development as a Marriage and Family Therapist
    • Protect client welfare
    • Evaluate the trainee's readiness for independent clinical practice
  • Risks and Benefits of Supervision. Benefits include professional growth, skill development, ethical guidance, and clinical accountability. Risks may include emotional discomfort related to feedback, evaluation, or gatekeeping decisions. The trainee acknowledges that supervision is evaluative in nature and not therapy.

  • II. PRAGMATICS AND LOGISTICS

  • Financial Arrangements
    Supervision fees are $90 for a one-hour individual supervision session, $120 for a 1.5-hour session, and $180 for a two-hour session. All fees are payable in advance through the designated online payment platform and are due at the time of booking or prior to the supervision session. Cancellations made with less than 24 hours’ notice are considered late cancellations and are non-refundable.

  • Supervision Structure and Frequency.

  • The standard supervision session is one (1) hour. Extended supervision sessions of 1.5 and/or 2 hours may be approved at the Supervisor’s discretion on a case-by-case basis and should not be construed as routine or ongoing supervision unless agreed upon. 

  • Documentation Requirements. The trainee is responsible for maintaining accurate clinical and supervision hour logs, completing progress notes in accordance with legal and ethical standards, and retaining supervision documentation required by training programs or licensing boards. The Supervisor will verify hours only when documentation is complete and accurate.

  • Evaluation Procedures. Evaluation is both formative (ongoing feedback) and summative (formal assessment). Methods may include supervisor observation, review of recordings and case notes, written evaluations, and self-assessment. Unsatisfactory progress may result in remediation, increased supervision, or termination.

  • Termination of Supervision. Supervision may be discontinued due to ethical violations, failure to meet competency standards, noncompliance with supervision requirements, or mutual agreement. Reasons for termination may be disclosed to training programs, subsequent supervisors, or licensing boards for gatekeeping purposes.
  • III. METHODS OF SUPERVISION

  • Supervision will be conducted 100% virtually using a HIPAA-compliant telehealth platform. Supervision may include case presentations, audio/video recordings, live observation, and technology-assisted supervision. The trainee agrees to provide access to clinical material as required. All materials must comply with HIPAA and state confidentiality laws.

     

     

  • IV. RESPONSIBILITIES AND EXPECTATIONS

  • Supervisor Responsibilities. The Supervisor agrees to provide competent, ethical supervision, maintain appropriate boundaries, provide timely feedback, maintain confidentiality within ethical and legal limits, and give reasonable notice of cancellations. The Supervisor will also verify, document, and sign all clinical hours and required forms for MFT licensure that have been collaboratively reviewed and agreed upon by both the Supervisor and Trainee.

  • Trainee Responsibilities. The trainee agrees to be on time and prepare adequately for supervision, notify the Supervisor promptly of cancellations, seek supervision for ethical or legal concerns, practice within scope of training, and inform clients that services are provided under supervision. Trainee will complete required forms and/or videos prior to supervision sessions. Trainee will complete all client documentation required by the supervisor, agency, and others as indicated in the policies and procedures manual and this contract. 

  • V. AGREEMENTS REGARDING SUPERVISOR NOTIFICATION

  • The trainee agrees to inform the Supervisor immediately (within 24 hours) regarding client crises, suicidal or homicidal ideation, threats of violence, ethical complaints, suspected abuse or neglect, boundary concerns, or clinical errors. The trainee agrees to be honest and forthcoming about any ethical or legal matters and report immediately (within 24 hours) any mistakes that could jeopardize or harm client care.

    Mandated Reporting and Vulnerable Populations
    The Trainee acknowledges that they are a mandated reporter under state law for suspected abuse or neglect of children, dependent adults, or vulnerable adults. The Trainee agrees to:

    1. Immediately report any suspected abuse or neglect to the appropriate authorities in accordance with state law.
    2. Notify the Supervisor immediately after making or prior to making a report. The Supervisor will provide guidance on documentation, safety, and next steps.
    3. Maintain accurate, objective documentation of all disclosures and reporting actions.
    4. Follow ethical standards, AAMFT Code of Ethics, and agency policies regarding the care and protection of vulnerable populations. Click Here for Code of Ethics
    The Trainee understands that these steps are legally and ethically required and are separate from supervision or any other responsibilities under this Contract.

  • Trainee's Placement Information

  • As your supervisor, I may communicate with your placement director, clinical advisor, or on-site supervisor as needed to support your training and professional development. I will always inform you in advance whenever such communication takes place.

  • VI. CONFIDENTIALITY

  • The Trainee agrees to maintain HIPAA compliance and all applicable federal and state confidentiality laws in all aspects of clinical practice and supervision, including record keeping, client communications, telehealth services, electronic storage, and technology-assisted therapy and supervision.

    All client information and supervisory material shared in supervision shall remain confidential and used solely for supervision and training purposes, with limits to confidentiality as required by law, ethical standards, and gatekeeping responsibilities. Failure to maintain confidentiality or HIPAA compliance may result in corrective action or termination of supervision.

  • VIII. ADDITIONAL POLICIES

  • Supervision is distinct from therapy. Personal therapy may be recommended but not required. Multiple relationships will be avoided or ethically managed.

    Ongoing Documentation: 1x per month

    The Supervisor will conduct monthly case reviews, typically near the end of each month, for clinical evaluation, professional development, and verification of accrued supervision and clinical hours.

    As part of the review, the Trainee agrees to submit one de-identified progress note, including the client’s first and last initials, session date and number, at least one treatment plan goal, at least one intervention, client contact information, and identification of any ethical, legal, safety, or mandated reporting concerns, along with relevant social and cultural considerations, a genogram when appropriate, and the Trainee’s clinical conceptualization and the client’s stated goals for therapy.

    The monthly review will include an informal case consultation, discussion of the Trainee’s strengths and areas for growth, and calculation and verification of monthly hours. With documented client consent, the Trainee is encouraged to provide a 10–15 minute audio or video recording of a session. Submission of recordings is optional but strongly recommended, particularly given that supervision is conducted virtually.

    All materials reviewed will remain confidential and used solely for supervision and evaluation purposes. This information will be submitted via the FWT website.

    Ongoing Documentation: 1x every 4-6 months

    The Supervisor will conduct a formal written evaluation every 4–6 months to provide feedback on clinical skills, professional development, and supervision goals.      Social Media and Online Privacy Policy

    The trainee agrees to the following regarding social media and online interactions:

    No Client Contact Online: The trainee will not accept friend requests, follow requests, or any form of personal connection from current or former clients on social media or other online platforms.


    Confidentiality: The trainee will not share any client-identifiable information, case details, or therapeutic content on social media, blogs, or other online forums.


    Online Mental Health Groups: The trainee will not join mental health-related online peer groups or forums where client confidentiality could be compromised.


    By signing this agreement, the trainee acknowledges the importance of maintaining professional boundaries and client confidentiality in all online interactions.

     

  • IX. DUE PROCESS AND DISPUTE RESOLUTION

  • Due Process and Consequences for Errors
    The purpose of due process is to ensure that any concerns regarding the Trainee’s clinical or professional performance are addressed fairly, transparently, and constructively.
    1. Identification of Concerns:

    If the Supervisor observes errors, mistakes, or concerns in clinical practice, documentation, or adherence to ethical, legal, or professional standards, the Trainee will be notified promptly and provided with specific feedback.
    2. Discussion and Remediation:

    The Supervisor and Trainee will engage in discussion, reflection, and planning to address the issue. This may include:

    Additional supervision or observation
    Review of relevant training materials or ethics codes
    Targeted skill development or practice
    3. Documentation:

    All discussions, feedback, and agreed-upon remediation steps will be documented in supervision records.
    4. Repeated or Serious Concerns:

    If mistakes or concerns occur multiple times or involve serious ethical, legal, or safety issues, the following may occur:

    Formal written remediation plan
    Increased supervision frequency
    Temporary suspension of clinical activities until issues are addressed
    Recommendation for additional training or coursework
    Termination of supervision if concerns are unresolved or constitute risk to client welfare, legal compliance, or professional ethics
    5. Trainee Rights:

    The Trainee has the right to respond to feedback, ask questions, and collaborate on a remediation plan. All actions taken will be fair, documented, and aligned with AAMFT ethical and professional standards.

  • X. DURATION AND TERMINATION

  • This Contract is effective for one (1) year from the date of signing, unless renewed, amended, or terminated earlier in accordance with the terms outlined herein. It may be renewed, revised, or terminated according to the criteria outlined above.

  • XI. ACKNOWLEDGEMENT AND SIGNATURES

    The first session will review the contract, supervision expectations, and evaluation process. The second session will address supervision goals, state licensure requirements, and the licensure application process.

    A summary table of supervision requirements for the states in which the supervisor is licensed is provided for reference only. The supervisee is responsible for remaining informed of all current licensure laws and requirements applicable to their state(s).
