    This is your official team registration. Your payment will be required within 28 days of submitting. All player details are required. Players will need to ensure they have a copy of their passport or birth certificate as FTA can request proof of age at any time.

  • Important Notes - Early Bird Special (save $100 if you register before 30th September)

    You are to supply your own uniforms, every player must be in the same full uniform

    Players must present ID at any time to take the court if requested by FTA or staff 

    Every player who takes the court is covered under the TEAM conditions, code of conduct and policies.

    There must be a minimum of 6 players per team and maximum of 12 players per team. No more than 12 players can take the court for one team during the event. 

    Team Fee: $1000, must be paid by due date or late fee of $100 will be added. 

    Registrations will close once divisions are filled. There is a limit to court space and therefore limit of teams in each division. 

    There will be no refunds given.

    We will operate under FIBA rules, however, a DUB Cup Rules will be release with slightly different timings and rules relevant to operations required for the event.

  • Commitment of Payment

    As the primary contact, I understand I am to pay the invoice by the due date and late payment will result in a late fee. If I withdraw my team after the due date, I am still obliged to make payment as costs to FTA are still applied. I understand that failure to pay on time may mean my team loses their position in the compeition. I also understand no refunds will be given for any reason should i withdraw from the competition. 

  • Release of Liability

    Although the safety of all sport activities is the primary concern, indoor sport activities at FTA's facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Sport Center and the persons in charge. This also extends to activities within the overall tour if I partiicpate within any activity organised by the event managers (FTA or PlayWell Event & Travel)

  • Photo and Video Consent

    I give permission for all players who play for my team has consent for the use of photos and videos to be used by FTA during the event and beyond. This includes adveritsing, social media and more. This is ongoing 

  • Media or Public Posting

    I acknowledge that I must mention/acknowledge Fortitude Training Academy and The DUB Cup  when posting about the event and any partiicpant within my team. The conduct on any public forum will not be negative toward the event or activities involved. This may lead to disciplinary actions by event organisers

  • Conduct, Terms and Conditions

    I acknowledge and accept on behalf of my team and anyone associated with the teams, to follow the code of conduct and the terms and conditions set out by FTA found on their website. This includes the notes stated above in this form
