Try-OUT will be on Friday March 27, 7pm and Sunday March 29, 12pm

Ages 16 and up, if you are younger (14/15) and wish to be a part of our program please contact Coach Erly Cornejo or President Milton Carrera

Cost $40. After submitiing this form you will be redirected to a payment page PLEASE make sure to finalize that step to complete your registration. The Fee is required to participate in the Try-out.

Informed Consent and Acknowledgement:

WAIVER: I hereby give my approval to participate in any and all activities prepared by Sparta FC during the selected Tryout. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Sparta FC. and all its respective officers, agents, and representatives from any and all liability for injuries while participating induring the tryout.

In case of injury, I hereby waive all claims against Sparta FC. including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
PARENTAL CONSENT:

I am the minor’s parent or guardian named above and I understand the nature of the Waiver of Liability above and verify and consent to the minor attending Sparta FC Tryouts. On the minor’s behalf, I hereby release, covenant not to sue, discharge, and hold harmless Sparta FC, its volunteers, coaches, employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to Sparta FC, its volunteers, coaches, employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending in any Sparta FC. I also understand the camp retains the right to use publicity and advertising purposes photographs of players at the event.
