This Liability Waiver and Release of Claims ("Waiver") is entered into by the undersigned ("Participant"), on behalf of themselves and/or as the legal owner, foster, or authorized representative of the dog(s) identified below, in connection with air transportation services provided by K9 Coop Inc. and the pilot(s) operating the aircraft.

The Participant acknowledges that K9 Coop Inc. coordinates air transportation and, in limited circumstances, air rescue services for dogs, which may involve private or chartered aircraft operated by a properly licensed pilot. The purpose of these services is to relocate dogs between shelters, rescue partners, foster homes, adopters, or veterinary facilities.

The Participant understands and acknowledges that air transportation involves inherent risks, including but not limited to turbulence, weather conditions, mechanical issues, handling during loading and unloading, and stress-related reactions by animals. The Participant voluntarily assumes all such risks, whether known or unknown, associated with the air transport of the dog(s).

The Participant represents that, to the best of their knowledge:

• The dog(s) are medically fit for air travel.
• All required vaccinations, health certificates, and documentation are accurate and up to date.
• Any known medical conditions, behavioral concerns, or special handling requirements have been fully disclosed to K9 Coop Inc. prior to transport.

To the fullest extent permitted by law, the Participant hereby releases, waives, and forever discharges K9 Coop Inc., its officers, directors, employees, volunteers, agents, contractors,

and the pilot operating the aircraft, from any and all claims, demands, actions, damages, losses, costs, or liabilities arising out of or related to the air transportation of the dog(s), including but not limited to injury, illness, escape, delay, or death of the dog(s), except in cases of gross negligence or willful misconduct.

The Participant agrees to indemnify and hold harmless K9 Coop Inc. and the pilot from any claims, liabilities, damages, or expenses (including reasonable attorneys' fees) arising from:

• Inaccurate or incomplete information provided by the Participant;
• The dog's behavior before, during, or after transport;
• Claims brought by third parties related to the dog(s).

The Participant understands that while all reasonable precautions are taken to ensure safe and humane transport, no guarantees are made regarding flight conditions, timing, outcomes, or the dog's response to air travel.

The Participant affirms that they are the legal owner of the dog(s) or have full authority to execute this Waiver on behalf of the owner or responsible organization.

This Waiver shall be governed by and construed in accordance with the laws of the state in which K9 Coop Inc. is incorporated, without regard to conflict-of-law principles.

By signing this document electronically, the Participant acknowledges and agrees that their electronic signature is the legal equivalent of their handwritten signature. The Participant consents to conduct this transaction electronically and understands that this Waiver may be executed and retained in electronic form.
