By submitting this form, I acknowledge and agree that the information I provide may be shared with The Reunite Tonight Project and the Chesapeake Humane Society for the purpose of the program participation and related services. 

I understand that Virginia law requires all dogs and cats four (4) months of age and older to be vaccinated against rabies by a licensed professional. 

I further acknoweldge that The Reunite Tonight Project is not responsible for the cost of rabies vaccination or any other medical care, and that program assistance is limited to the microchip and microchip implantation only, unless otherwise expressly stted in writing.
