Love Dipaways | Cancellation Or Change Authorization Form

Please complete this form in order to process a cancellation or change for your trip. In order to process a cancellation for your trip there is a required CANCELLATION FEE of $50 per person and a $50 per person CHANGE FEE as noted in our Terms and Conditions on our booking site. Please note that there may be additional charges or fees required by the supplier which can be found on the supplier's website. Once the form and fee has been received you will receive an invoice with the fees listed from us and the supplier (if any) to process your change or cancellation.

Please note that No booking/reservations can be processed for cancellation or changes until an authorization form and the fees have been received. Please make sure to review the terms and conditions found on our website to learn more about the policy regarding cancellations and or changes.

Vacation Cancellation or Change Information

I understand that I may see charges from LOVE DIPAWAYS and their Travel Partners and that records will be provided to me to confirm the payments I have submitted or authorized. 

I, the above-named Authorized Credit Card User, give Love Dipaways authorization to charge my credit card for the purposes detailed above. I understand that this form is a legally binding contract and that by affixing my signature to this form, I will be held responsible for all agreed upon charges. This credit card is authorized for the charges noted above and any charges provided by the supplier. I also understand that there may be additional charges or fees required by a vendor in order to cancel or make changes to my reservation.

Note: Your card will not automatically be charged by completing and submitting this form for the amount and purpose authorized above but it does allow Love Dipaways to charge the payment method provided on the invoice that you will be sent.
