BPE and Mr. Boyer are proud to offer physical education based summer camps for elementary age students. Each camp will have a unique theme and will be designed to be fun and challenging for all age groups and levels of ability.

For Participants Under Age 18 - Parent/Guardian Consent and Waiver

acknowledge and agree that:

1. There is significant risk of injury while participating in BPE PHYSICAL EDUCATION LLC programs or events, ranging from minor to fatal, whether caused by me, another, or others. While rules, equipment and personal discipline may reduce risks, the risk of serious injury does exist.

2. I KNOWINGLY, VOLUNTARILY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, related to: a) my presence and/or participation in any BPE PHYSICAL EDUCATION LLC programs, events and activities, whether conducted at Trinity Christian  Fellowship or an outside location; and b) my use of any equipment at or belonging to BPE PHYSICAL EDUCATION LLC, even if arising from the negligence of the Releasees or others, and assume full responsibility for my participation and use.

3. I represent that I am capable of participating in BPE PHYSICAL EDUCATION LLC activities, events and programs, and take full responsibility for not exceeding my limits.

4. I willingly agree to comply with the stated and customary terms and conditions for participation in BPE PHYSICAL EDUCATION LLC programs and use of BPE PHYSICAL EDUCATION LLC equipment.

5. I hereby authorize the staff of BPE PHYSICAL EDUCATION LLC to act for the applicant according to their best judgement in providing or arranging for emergency care in any emergency circumstance requiring medical attention.

6. I, for myself and the minor(s) noted, hereby authorize the staff of BPE PHYSICAL EDUCATION LLC to provide vehicular transportation to and from outside locations depending on the activity. 

7. I understand that BPE PHYSICAL EDUCATION LLC does NOT provide medical insurance and that I am responsible for all medical expenses resulting from my child or ward’s participation in the Event. In the event I cannot be reached in a medical emergency, I hereby agree to grant permission to the program medical staff to act on my behalf in case of a medical emergency.

8. I, for myself and the minor(s) noted, and on behalf of our respective personal representatives, heirs, next of kin, spouse and assigns, HEREBY KNOWINGLY AND INTENTIONALLY RELEASE, INDEMNIFY AND HOLD HARMLESS BPE PHYSICAL EDUCATION LLC, their owners, officers, directors, members, employees, officials and agents, other participants, sponsoring agencies, sponsors, advertisers, and owners and lessors of premises used by BPE PHYSICAL EDUCATION LLC to conduct its activities and programs (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

9. I consent to BPE PHYSICAL EDUCATION LLC taking photographs, digital images, video recordings in documenting the activities of BPE PHYSICAL EDUCATION LLC programs, events, activities and services. I grant to BPE PHYSICAL EDUCATION LLC and its affiliates a perpetual, fully-paid up, world-wide license to use the digital images, prints, motion pictures, video and audio recordings or any other reproduction of the same for BPE PHYSICAL EDUCATION LLC and its affiliates' educational and promotional purposes in such forms and on such modes of communication as BPE PHYSICAL EDUCATION LLC sees fit.

10. I understand that the weather or natural disasters can play a significant roll in the success of a class. In the event of inclement weather or a natural disaster, classes may be cancelled and unable to be rescheduled. In the event of cancellation because of inclement weather or natural disaster, refunds will not be given.

I, as the parent or guardian with legal responsibility for this participant, consent and agree to his/her release as provided above of all the Releasees, and, for myself, heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in BPE PHYSICAL EDUCATION LLC programs or equipment use as provided above, even if arising from the negligence of the Releasees, to the fullest extent permitted by law.
