All youth under the age of 18 MUST be dropped off and picked up by a parent or guardian unless otherwise stated below.

I, {legalParentguardian}, acknowledge that I am aware of, appreciate the character of, and knowingly and voluntarily assume the risks involved in participating in all activities. On behalf of myself: my heirs, next of kin, successors in interest, assigns, personal representatives, and agents, I hereby:

1. Acknowledge that these activities may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include but are not limited to those caused by terrain, facilities, equipment, lack of hydration, and actions of other people, including, but not limited to, participants, volunteers, monitors, and/or producers of the activity.
2. Waive and release any claim or cause of action against and release from liability Cape Assist, its officers, employees, and agents for any liability for injuries to my person or property resulting from my use or participation in the activity listed above, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entitie·sbeing released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
3. Certify that I am physically fit and have not been advised by a qualified medical professional not to participate in the activity. I certify that there are no health-related reasons or problems which preclude my participation in these activities;
4. Agree to indemnify and hold harmless Cape Assist, its officers, employees, and agents for any claims, causes of action, or liability to any other person arising from my use of the facility or participation in the activity listed above;
5. Acknowledge a signature by the parent or legal guardian of any minor child participant is required.
   
   

I, {legalParentguardian}, HAVE READ THIS ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND RELEASE AGREEMENT. I FULLY UNDERSTAND ITS TERMS, UNDERSTAND I HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME, AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY NEW JERSEY LAW.

I, {legalParentguardian}, hereby grant and authorize Cape Assist the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or videos taken of me and/or my child by a representative of Cape Assist to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely unless I otherwise revoke said authorization in writing.

I understand and agree that it is my responsibility to immediately notify Cape Assist if any minor participants I have legal authority over wish to decline being photographed or videoed during the event/program.

I understand and agree that these materials shall become the property of Cape Assist and will not be returned.

I hereby hold harmless and release Cape Assist from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or behalf of my estate.

I, {legalParentguardian}, understand and agree to follow the policies, guidelines, and/or rules set forth by Cape Assist and the designated venue outline below, in addition to items that may be verbally communicated without prior written notice. In addition, I will ensure that all participants attending the event/program for whom I have legal authority understand and follow the items below, in addition to any items that may not be listed but set forth prior to or during the event by Cape Assist.

• The use of alcohol, drugs, tobacco, or vaping products during the event is prohibited, whether inside or outside the venue where the Youth and Family Skate Night Event is taking place.
• Any individual who appears to be under the influence of any substance will not be permitted entry to the event or venue or will be asked to vacate the property immediately.
• Individuals must follow all venue and event rules, whether they are posted or communicated verbally by a staff member or authorized representative.
• Any harassment or bullying of another individual will not be tolerated, and the individual may be asked to leave.
• A minor must be accompanied by a legal guardian at all times.
• The use of any inappropriate language, gestures, clothing, or accessories is prohibited.
• Weapons of any kind are prohibited.

Cape Assist and the designated venue reserve the right to refuse entry or remove any individual and/or their party from the event/property if they are deemed to be inappropriate, disruptive, threatening, or not following the policies, guidelines, and/or rules.
