PARTICIPANT AGREEMENT AND RELEASE FROM LIABILITY

I, the undersigned , acknowledge that I have voluntarily applied to participate in a volunteer project through Come Closer and affiliated churches.

I recognize that there are risks involved in participating and hereby assume all risks including but not limited to: sickness, travel, transportation, as well as similar and dissimilar risks in connection with my participation in it. I understand and agree that neither Come Closer and affiliated churches nor their trustees, officers, directors, employees, agents or representatives may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation and hereby release Come Closer and affiliated churches, their trustees, officers, directors, employees, agents and representatives from any injury, harm, damage or death, which may occur while I am participating in the volunteer activities.

As consideration for being permitted by Come Closer and affiliated churches to participate and as a consideration for Come Closer and affiliated churches assisting in arranging the volunteer activities, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby irrevocably and unconditionally release, waive, discharge and covenant not to sue or attach the property of the Come Closer and affiliated churches or any of their affiliates, subsidiaries, mission agencies, mission affiliates, divisions, members, directors, officers, employees and agents (collectively referred to as the “Releasees”), for and from all claims of any nature now or hereafter existing whether known or unknown, including but not limited to all liability, on account of death, injury, or damage resulting from the negligence or other acts, however caused, of the Releasees as a result of my participation.

KNOWING AND VOLUNTARY EXECUTION

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF ON THE ONE HAND, AND COME CLOSER AND AFFILIATED CHURCHES, AND/OR THEIR AFFILIATES ON THE OTHER HAND. NO ORAL REPRESENTATIONS, STATEMENTS OR INDUCEMENTS APART FROM THIS AGREEMENT HAVE BEEN MADE TO ME. I SIGN THIS AGREEMENT OF MY OWN FREE WILL.
