I, the undersigned, (parent or guardian if participant is under 18), attest that the information provided herein is accurate and complete to the best of my knowledge. I accept legal and financial responsibility for: a) any material misrepresentation included herein and b) any expenses incurred related to participation at Camp Christian, including medical expenses. I give permission for the above named "Adult Leader from Church" or any Camp Christian staff or The Armory member to transport me/my child to and from camp and/or during camp as necessary and understand that the release and indemnification provided in the document referenced below extends to transportation of me/my child. I acknowledge that by signing this document, I a) authorization for the adult leader from the church listed above to seek medical treatment for me (or my child) in the event I cannot give consent, b) indemnification for Camp Christian and The Armory from liability related to damages or injuries of any kind sustained because of participation in this event, and c) discretion for the director of camp to remove me (or my child) from camp if in their opinion my/his/her presence poses a threat to myself/himself/herself or anyone else at camp. I understand and consent that the Participant will share a sleeping room-but not a bed-with 1 or more members of the same sex and that in some cases the same room may have both adults and students staying in it.

Airsoft Activity: Parental Permission - Release

To participate in Airsoft Games and other youth activities at Camp Christian, 2915 Co Rd 241 New Castle, CO in connection with the Outdoor Skills Camp presented by The Armory. I understand that this activity may involve some risk of injury to my child, either in connection with the activity itself, or in connection with travel to and from the activity, and the team building work performed in the sport of Airsoft Games. I acknowledge the fact that attendance to this activity is being offered to me and to my children without cost. For myself and my child, I hereby assume any and all risk of injury to my child arising out of participation in this activity. In the event any claim is ever made against The Armory, Camp Christian, or against and of its members, leaders, agents, and any person assisting them with youth activities, for any injury suffered by my child, I hereby agree to indemnify, defend and hold The Armory, Camp Christian, or any associated persons harmless from any liability connected with such claim.

I also give permission to those assisting in this youth activity to obtain emergency medical attention for my child(ren) in case of injury. In the event I cannot be reached in an emergency, I authorize the leaders of the youth activity to consent in my place to such treatment as may be necessary for my child.
