Photo & Media Release

By checking "Yes," above, you hereby grant Petrover Orthodontics and its authorized representatives the irrevocable right to use, publish, and copyright any photos or videos submitted by you or featuring you (and/or your minor child listed in this form).

Usage Rights You authorize the use of these images in any media now known or later developed—specifically including the practice website, Google Business Profile, and social media pages—for advertising, marketing, or any other professional purpose. This includes the right to edit, alter, or combine these images with other materials.

Waiver & Release You hereby waive any right to inspect or approve the finished product or the specific advertising copy used. Additionally, you release Petrover Orthodontics from any and all claims or liability arising from the use of these images, including but not limited to claims for blurring, alteration, libel, or invasion of privacy.

Legal Authority You warrant that you are of legal age and have the right to contract in your own name, or that you are the legal parent or guardian of the minor(s) named below. You have read this release and fully understand its contents.
