  • 1. PURPOSE OF THIS AGREEMENT

  • Brinkley Haven is a community-based independent and co-living residence designed to provide safe, comfortable housing and optional supportive services in a residential setting.

    This Agreement outlines the housing arrangement, community expectations, and non-medical services provided by Brinkley Haven.

    Brinkley Haven does not provide medical care, skilled nursing, licensed assisted living services, or clinical care of any kind.

  • 2. HOUSING & COMMUNITY LIVING

  • Brinkley Haven agrees to provide the Resident with:
    • A furnished private or shared bedroom (as selected)
    • Access to shared living areas, dining areas, and outdoor spaces
    • Utilities including electricity, water, trash, and high-speed internet
    • A clean, respectful, community-oriented living environment
  • Room assignments are based on availability and may be adjusted for operational or safety reasons with reasonable notice.
  • 3. SERVICES INCLUDED (NON-MEDICAL)

  • The following non-medical services are included:
    • Housekeeping of common areas and resident room
    • Scheduled laundry services
    • Meal coordination or meal plan (if selected)
    • Community activities and companionship
    • Daily general wellness check-ins (non-clinical)
    • Transportation coordination (when available)
  • Brinkley Haven does NOT provide:

    • Medical or nursing services
    • Therapy or clinical services

    Residents may independently contract with licensed third-party providers, including home health, hospice, or medical professionals, at their own discretion and expense.

  • 4. OPTIONAL TRANSPORTATION SERVICES

  • Transportation services may be provided for non-emergency purposes only, including doctor appointments, errands, outings, and community activities, based on availability. 

    • $50 per hour
    • One (1) hour minimum
    • Time calculated from departure to return
    • Transportation fees are billed separately and are not included in the monthly housing fee
    • Emergency or medical transportation services are not provided.
  • 5. RESIDENT RESPONSIBILITIES

    The Resident agrees to:
    • Conduct themselves in a respectful and cooperative manner
    • Follow all house rules and community policies
    • Maintain personal cleanliness and hygiene to the extent possible
    • Treat staff, residents, and property with respect
    • Promptly notify Brinkley Haven of changes that may affect safe community living
  • 6. Resident Admission Fees and Payment Terms

    •  Application Fee
      $200 non-refundable application fee due at the time of application
      Submission of an application does not guarantee acceptance
      Resident Admission Fee (Non-Refundable)

    • $1,500 one-time non-refundable fee due prior to move-in
      Covers administrative setup, onboarding, room preparation and orientation
      (Non-Refundable)

  • Late Fees

    Monthly payment is due on the 1st day of each month.
    • $50 per day for payments not received by the due date
    • Continued non-payment may result in termination of this Agreement with proper notice
  • 6a. Accepted Payment Methods:

    Brinkley Haven accepts debit/credit card payments and cashier’s checks.For security purposes, personal checks are not accepted.
  • 7. NO REFUNDS / EARLY MOVE-OUT POLICY

  • All fees paid to Brinkley Haven are non-refundable, including but not limited to:
    • Application Fee
    • Resident Admission Fee
    • Monthly rent
    • Any prepaid housing or service fees
  • If the Resident vacates the premises early, relocates, is hospitalized, or leaves for any reason after occupancy begins, no refunds or prorated credits will be issued for unused days or partial months. 

    Once occupancy has begun, rent and fees for the current month remain due and payable in full, regardless of early move-out or absence. 

  • 8. EVACUATION DEPOSIT (REFUNDABLE)

  • A $3,000 Evacuation Deposit is required prior to move-in and must be submitted in the form of a check or cashier's check payable to Brinkley Haven. This deposit is held solely for emergency evacuation purposes, including but not limited to hurricanes, severe weather events, or other natural disasters requiring relocation. If an evacuation becomes necessary and the Resident evacuates with Brinkley Haven, this deposit may be applied toward transportation, temporary lodging, food, staffing, and related emergency expenses. If no evacuation occurs, the Evacuation Deposit will be refunded in full. If evacuation costs are incurred, any unused portion will be refunded after final reconciliation.
  • 9. TERM & TERMINATION

  • This Agreement is month-to-month. Either party may terminate this Agreement with 30 days' written notice. Brinkley Haven reserves the right to terminate this Agreement immediately if:
    • The Resident poses a risk to themselves or others
    • Community rules are repeatedly violated
    • The Resident's needs exceed what Brinkley Haven can safely support
    • Non-payment occurs
  • 10. HOUSE RULES

    • Visiting Hours: 10:00 AM – 6:00 PM
    • Quiet Hours: 8:00 PM – 6:00 AM
    • Smoking: Not permitted
    • Pets: Not allowed
    • Alcohol & Illegal Drugs: Strictly prohibited
    • Shared Areas: Must be kept clean and respectful
    Violation of these rules may result in termination of residency.
  • 11. SAFETY & EMERGENCIES

  • Brinkley Haven maintains emergency and evacuation procedures. In an emergency, staff may contact emergency services or the Resident's emergency contact. Brinkley Haven is not a medical facility and does not assume responsibility for medical outcomes.
  • 12. ABUSE & NEGLECT NOTICE

  • If you suspect abuse, neglect, or exploitation of an elderly or disabled person, report immediately to:

    Available 24 hours a day, 7 days a week.
    Retaliation for good-faith reports is prohibited.
  • 13. EMERGENCY CONTACT & RESPONSIBLE PARTY

  • Responsible for payments and/or decisions if Resident is unable to do so
  • 14. GUARANTOR / GUARDIAN (IF APPLICABLE)

  • The Guarantor agrees to be financially and legally responsible for the Resident under this Agreement.
  • 15. SIGNATURE

  • By signing below, the Resident and/or Responsible Party acknowledge that they have read, understand, and agree to all terms of this Agreement.
