📝 Nature of Camp Participation
Enrollment in the camp grants the participant access to the assigned classes and activities based on the selected program and the current schedule at Champions Sports Club (NOLA CHAMPION LLC).
Participation in the camp does not grant the participant or their parent/guardian any ownership rights or decision-making authority over NOLA CHAMPION LLC, its facilities, operations, or management.

💰 Cost   

.       Full Week Registration (5 days) First Child- $280

.       Sibling Full Week Registration (5 days) Each additional sibling- $220 

(Sibling discount applies only when the first child is also registered for the full  week)

.       Short Week Registration (4 days) - $240

·       One-Day Registration: $70

🔁 Refund and Change Policy

We follow a strict no-refund policy once payment for the Champions Sports Camp has been made. Champions Sports Camp refers to all camp programs offered by NOLA CHAMPION LLC. After registration and payment, the spot is considered non-refundable, regardless of the reason for absence. Our camps have very limited capacity, and we plan staffing, materials, and resources in advance based on final registrations. We kindly ask for your understanding and commitment when registering.

⚠️ Exception — Early Change Notice

If you notify us of a change in plans at least thirty (30) days before the scheduled Summer Camp day, we may, at our discretion, offer to transfer your child’s registration to another available date during Summer Camp 2026.

- This is not guaranteed and is subject to availability.

- No refunds will be issued if a suitable alternative date is not available.

All requests for date changes must be submitted in writing via email to championnola@gmail.com

Requests submitted in person, by phone, or after the deadline will not be accepted. We truly appreciate your understanding and cooperation.

📅 Schedule & Program Changes
Drop-Off: 8:00 – 8:30 AM
Pick-Up: 2:30 – 3:00 PM

NOLA CHAMPION LLC reserves the right to adjust the camp schedule, program structure, activities, and assigned instructors at any time — based on staff availability, weather conditions, or other organizational needs.
While we strive to provide consistency, some activities may be modified, combined, or replaced to ensure a safe and high-quality experience for all children.
In the case of unforeseen circumstances (such as hurricanes, power outages, or emergencies), parents will be notified as early as possible. If feasible, an alternative plan may be offered.
Missed days due to such circumstances are not eligible for refunds or make-up credits.

🔴 IMPORTANT: No Before or After Care Provided
We do not offer before-care or after-care.
All children must be dropped off and picked up within regular camp hours.
⏰ Late Pick-Up Fee: $1 per minute, per child
A late fee will be charged for any child picked up after 3:00 PM.
Payment for late pick-up must be made on the same day via:
·       Cash
·       Zelle
·       Venmo
Example: If you pick up two children 10 minutes late, the late fee is $20.

🙋‍♂️ Personal Conduct
We ask all children to be respectful, kind, and follow instructions from our coaches and staff at all times.
Safety, kindness, and teamwork are our top priorities.
By enrolling your child, you agree that:
·       Your child will follow the rules and behavior expectations shared by the instructors.
·       You will speak with your child about behaving respectfully and safely during all activities.
·       If your child’s behavior puts others at risk, you may be contacted and asked to pick them up.
·       You are financially responsible for any damage caused by your child’s inappropriate or reckless behavior (to equipment, facility, or others).
We do everything to create a positive and safe environment — and we thank you for your support in making it successful.

🦠 Health Risks & Illness Disclaimer
I understand that participation in group activities may involve the risk of exposure to contagious illnesses, including but not limited to COVID-19, flu, or other viruses.
While NOLA CHAMPION LLC follows general hygiene and safety practices, I acknowledge that exposure is still possible and knowingly accept this risk for myself and/or my child.
I agree to keep my child home if they show signs of illness (e.g., fever, cough, vomiting, rash), and I understand that the staff may request early pick-up if symptoms appear during the day.
I release NOLA CHAMPION LLC and its staff from any liability related to the unintentional transmission of illnesses during program participation.

📸 Media & Model Release
I hereby grant permission to NOLA CHAMPION LLC to use photographs, video, audio recordings, or other media that may contain the image, name, or voice of myself and/or my child(ren), for the purposes of promotion, marketing, or informational materials.
This includes but is not limited to:
·       Social media
·       NOLA CHAMPION LLC website
·       Printed flyers
·       Newsletters
·       Online advertising
I understand that no compensation will be provided for the use of such media.
If I wish to opt out, I understand that I must notify NOLA CHAMPION LLC in writing via email to:📧 championnola@gmail.com before the camp begins.

🖊 Confirmation & Legal Authority
By checking the box below and signing this form, I confirm the following:
·       I have read this entire waiver and fully understand its terms.
·       I am signing voluntarily and acknowledge that I am giving up certain rights.
·       I agree that this waiver is legally binding to the fullest extent permitted by law.
·       I confirm that I am the parent or legal guardian of the minor participant(s) listed and have the legal authority to sign this waiver and consent on their behalf, including all terms regarding participation, media release, and liability.
·       I believe my child is physically and emotionally fit to participate in the activities provided by NOLA CHAMPION LLC.
·       I accept all terms of this waiver on behalf of my child(ren), myself, and our legal successors.

💻 Agreement to Electronic Communication
·       I agree to communicate electronically with NOLA CHAMPION LLC regarding camp participation.
·       I authorize notices, forms, and disclosures to be sent by email, and I understand that my electronic signature has the same legal value as a handwritten one.

💵 Financial Terms
·       I agree to all camp rules and policies, including those regarding:
o   refunds,
o   missed attendance,
o   late pick-up fees.
·       I understand that all payments are final and non-refundable, and a reserved spot remains valid regardless of attendance.
·       I agree not to dispute charges or take legal action related to registration, absence, or participation in any program of Champions Family Sports Club.

🔒 Confidentiality
All terms of this agreement are confidential and may not be disclosed to any third party, except to:
·       comply with applicable laws or court orders,
·       consult a legal, financial, or tax advisor,
·       or enforce the agreement.
This applies to both the parent/guardian and NOLA CHAMPION LLC.
