Tryout Permission & Agreement of Compliance

I have read the CISD Carroll Cheerleading Constitution and all tryout information and understand all requirements for participation in the cheerleading team. I also understand the dedication and time it takes to be involved in the Carroll ISD Cheer Program. I intend to devote the time needed to make the cheerleading team a success if I am selected. I understand that the decisions of the judges are final.

Our child has permission to try out for the Carroll Varsity or Junior Varsity Cheer Team. We have read the CISD Carroll Cheerleading Constitution and all tryout information, and we will assist in every way to see that these rules and policies are upheld should our child be selected for the team.

We understand that the financial obligations must be as set forth by the constitution. We also understand that the school and/or coaches and sponsors assume no responsibility for accident or injury that might occur during any cheerleading activity. I am satisfied with the high standards set forth to govern this organization. I understand that it is a privilege and not a right to be chosen to the membership of this group. I also understand that not everyone can be chosen. I further understand the tryout procedures and how the Carroll Cheerleaders are selected. I will agree to abide by the decision of the judges and I will agree that all decisions are final.

PARENTS: THE FOLLOWING RELEASE MUST BE SIGNED BY EACH PARTICIPANT’S PARENT OR GUARDIAN BEFORE THE TRYOUT PROCESS BEGINS.

A. I understand that by taking part in cheerleading, there is a possibility of injury or sickness to my son or daughter. I do hereby grant permission to hospital staff members to administer immediate treatment to my child should he or she be injured.

B. I also understand that the school and/or coaches and sponsors assume no responsibility for accident or injury that might occur during any cheerleading activity.

CARROLL INDEPENDENT SCHOOL DISTRICT DISCIPLINE POLICY STATEMENT

I understand and consent to the responsibilities outlined in the District’s Student Code of Conduct. I also understand and agree that my child shall be held accountable for the behavior and consequences outlined in the Standards for District Extracurricular Activities at school and at school sponsored or school district activities. This shall include, but not be limited to school sponsored as well as school related travel or activity, regardless of time or location.

Students who represent the district by wearing uniforms of the district shall be responsible for conducting themselves as outlined by the Standards for District Extracurricular

Activities and the Student Code of Conduct during the entire time they are wearing such uniform.

Students involved in extracurricular activities are expected to exhibit the highest standard of conduct. Carroll Independent School District views participation in extracurricular activities as a privilege and not a right. I understand that any student who violates the Student Code of Conduct will be subject to disciplinary action. This includes any misconduct, regardless of time or location that reflects negatively upon Carroll Independent School District in an extracurricular activity. Students involved in major disciplinary infractions (including but not limited to drugs, alcohol, and violent behavior) will be removed temporarily or permanently from extracurricular activities.

This form shall be returned to the designated sponsor and verified by the principal. Signatures must be obtained before any student may participate in extracurricular activities.
