Waiver
By submitting this form you;
1. I acknowledge that the attendance at any lesson, seminar, walk and train, board and train or group class held by canine impact and all employees may involve risk or injury or damage to; myself, other, my dog, or property, even when all due care is, and attention has been taken and subject to clause two below assume the risk involved.
2. canine impact acknowledges and understands that it will exercise all due care in carrying out the service in conformity with the fair-trading act and WHS rules and regulations.
3. in consideration canine impact and any premises on which a service is provide: a) except in relation to obligations under the fair trading and WHS rules and regulations, I otherwise release canine impact &/ or canine calm and all its employees, and contractors from any liability whatsoever for any injury or damage resulting in any way
4. You agree & understand that dogs are not to interact in classes unless working on a specific exercise to which dogs are to interact
5. I understand i am responsible for my dog and there safety at all times
6. I acknowledge i have ensured that canine impact and there training style is right for me and my dog and understand a refund will not be offered if i change my mind
7. I have read and understood the above document
