Owners Declaration
I confirm that I am the legal owner of the dog and hereby give my consent for it to receive massage and rehabilitation therapies. I have read and accept the terms and conditions of treatment, and I confirm that I have consulted with my dog's veterinarian regarding these therapies. To the best of my knowledge, there are no contraindications to massage or rehabilitation therapies for my dog.

Should the therapist determine during the session that further veterinary consultation is required, I understand that I will be informed, and the veterinarian will be contacted as appropriate.
