Member and spouse ONLY can complete this form/register any ATV/IUTV/golf cart.

As a member at Cowlitz Timber Trails Association, you confirm and understand your responsibilities:

1. You are the owner of the ATV/UTV/golf cart(s) listed on the back of this form.
2. You have read and understand Chapter 2 - Golf Carts/ATV/Motorbikes or Equivalent, and Vehicle Rules and Regulations. You will also review these rules with any family member or guest and understand that you are responsible for all family members and guests.
3. You are responsible for purchasing a CTTA assigned plate. The cost currently is $20.00. These are personalized and ordered. A temporary plate will be given to you at the time of registration and must be always displayed until the permanent plate arrives.
4. A yellow registration tag number is assigned at the time of registration and must always be visible on the ATV/UTV/golf cart.
5. An annual registration tag will be assigned to you and must be always displayed. Annual registration is due on June 30th of every year.
6. All of the vehicles listed carry personal liability limits of 100/300 and you have provided proof in the form of the policy declortation page prior to or at the time of registration that they are properly insured. This document must show the correct coverage amount. The document can not be a insurance card for the policy.
7. You are responsible for annually providing a Release of Liability form for every visitor that enters CTTA's gate. Age or whether they ride does not matter.
8. You are responsible for annually providing proof of liability insurance with 100/300 limits. If your policy is on a 6-month term, you must provide documentation at the time of renewal.
9. Any vehicle not insured/registered must be removed. Failure to remove will result in fees/fines, suspension, and action up to termination of membership.

By signing below, you confirm the information on this form and is true and accurate. You understand your responsibilities as an owner of any ATV/UTV/golf cart present and used on CTTA property. Using the lack of one's interpretation or understanding of the rules and guidelines for ATV/UTV/golf cart use is not an excuse for failure to follow the guidelines.
