Dear Parent/Guardian,

At Simcoe Spartans, we take pride in celebrating the effort, growth, and passion of our players. We love sharing moments of teamwork, skill, and fun through photos and videos captured during training sessions and events. These highlights may be shared on our official Simcoe Muskoka Soccer social media platforms, website, and other promotional materials to showcase our community and inspire others.

Please review the consent section below to let us know if we have permission to include your child in these features.

I grant permission to Simcoe Spartans (under Simcoe Muskoka Soccer) to take and use photos and/or videos of my child during training sessions and events for promotional purposes, including social media, website, and printed materials. I understand that no personal information will be shared without explicit consent and that I may revoke this permission at any time in writing.
Injury Waiver and Release of Liability

I understand that participation in Simcoe Spartans training involves physical activity and carries a risk of injury. I release Simcoe Spartans, Simcoe Muskoka Soccer, its organizers, coaches, volunteers, and facilities from any liability for injury, loss, or damage sustained during training activities. I accept full responsibility for any medical expenses that may arise from such injuries and confirm that my child is physically able to participate in outdoor soccer.
By selecting “I DO consent,” I acknowledge that I have read and agree to the program’s Waiver & Release of Liability and Media Consent. This includes accepting all risks of my child’s participation in the training program and granting permission for the use of my child’s image for promotional purposes. I understand I may withdraw media consent at any time by providing written notice to Simcoe Spartans / Simcoe Muskoka Soccer.*

To finalize your registration, payment ($180) is due April 4th, 2026. E-Transfer to:

Please include your child’s full name in the message.

After your registration is complete, you can expect a welcome text or call from Coach Sam to confirm everything and answer any questions.
