Please read before submitting form:

 

• You will be contacted via email for scheduling and deposit info within 2-5 days of submitting your form. (Please check your spam folder)

• A deposit is required to schedule and hold your appointment date(s). The deposit comes out of the total price of your last appointment. ($50 minimum deposit/$100 for larger pieces)

• If you do not reply within 48 hours of being contacted with my availability, your preferred time slot may be forfeited.

• Once you have chosen a date, you will be sent the deposit info promptly. All deposits must be submitted through Venmo.

• Deposits are NON-REFUNDABLE. If you do not show up to your appointment, arrive more than 15 mins late without notice, or cancel/reschedule within 5 days of your appointment time, your deposit will be forfeited. 
  
  
• An additional $50 deposit is required to reschedule if notified within 5 days of your appointment time; a $100 deposit is required upon a second consecutive reschedule
  
• After 2 reschedules (consecutive or not) a $150 deposit is required for all future appointments 

• I will not send your design before your appointment time. Once you are shown the design at the time of your appointment, we will collaborate in person if any changes need to be made. I am more than happy to modify your design and want you to end up with the exact tattoo you are envisioning! 
  

• I will not tattoo anyone under the age of 18
