Domestic competition: Independent Team Application Form

Winter 2026 season (U8-16)
This form is to be completed by new and existing Independent teams (U8-16) registering into the MBA junior domestic competition.

A SEPARATE REQUEST FORM IS TO BE COMPLETED FOR EACH TEAM REGISTERING

* Existing Independent teams: Any players who were not registered to the team in Summer 25/26 season must be approved by the MBA before joining the team.

* New Independent teams: All players must be approved by the MBA prior to registering to the team through PlayHQ.

NOTES:

* Clearance/Transfer Rules apply to all players (A maximum of two players can transfer from into an independent team from the one club in same division/age group/gender). refer to MBA ByLaw 8.

* Players/teams who do not comply with the MBA clearance ByLaws may be de-registered and/or omitted from their team by the MBA.

* Independent teams linked to a club that is registered with PlayHQ are NOT REQUIRED to complete this form.
