SKIN PACKAGE AGREEMENT

Purpose = Skin packages are designed to support long-term skin health and results. Because corrective treatments work cumulatively, packages require commitment, consistency, and compliance with professional guidance.

Benefits = When performed consistently and paired with proper home care, treatments may help improve different skin conditions & texture, clarity, tone, and overall skin appearance. Individual results vary based on skin condition, lifestyle, and compliance.

Confidentiality = All personal information, skin history, and treatment details shared are kept strictly confidential and used solely for providing safe, personalized services. Client information will never be shared without consent, except as required by law.

Risks = Skin treatments may involve temporary reactions such as redness, sensitivity, dryness, mild peeling, or purging. These responses are part of the skin’s natural renewal process and vary by individual. I acknowledge that following all pre- and post-care instructions is essential to minimize risks and support optimal results.

 

• All skin packages require a non-refundable deposit to secure appointments.
• Package balances are paid according to the agreed payment schedule.
• Deposits and package payments are non-refundable and non-transferable once services have begun.

 

 

• Appointments must be scheduled at least 10 days in advance when applicable (chemical peels, microneedling, corrective services).
• All package sessions must be used within the following timeframe:
  
  3-session packages: within 4-6 months
  6–9 session packages: within 6-9 months

Unused sessions after the expiration date are forfeited.

 

 

• A minimum of 72 hours notice is required to reschedule or cancel an appointment.
• Late cancellations or no-shows may result in the loss of that session.
• Repeated no-shows may result in termination of the package without refund.

 

 

Results depend heavily on client compliance, including:

Following ALL Pre - and Post -care instructions

Avoiding excessive sun exposure before and after treatments
Using recommended home care products

Failure to comply may affect results and does not warrant refunds or additional sessions.

 

 

Services within a package may be adjusted by the esthetician if necessary for skin safety.
Packages cannot be split between individuals or transferred.

 

The following deposits are required to secure skin package services. Deposit amounts are applied toward the total package investment and are non-refundable.

 

• Ultimate Detox Facial Series – $165 deposit
• Microneedling Series – $225 deposit
• Chemical Peel Series – $210 deposit
• Advanced Skin Transformation Series (9 Sessions) – $540 deposit

 

 

Deposits secure the client’s commitment and appointment availability. Remaining balances are due according to the agreed payment option below.

Clients may choose one of the following payment options for skin packages:

 

Option 1: Pay in Full
Remaining balance paid in full prior to or at the first session.

Option 2: Split Payments
Remaining balance divided evenly across scheduled sessions and paid at each visit.

 
All payments must be completed by the final session. Missed payments may result in paused services until the account is current.

• Total: $950

• Deposit: $210

• Remaining balance: $740

 

Payment schedule:

• Session 1 (deposit): $210

• Session 2: $370

• Session 3: $370

 

9-Advanced Skin Transformation

• Total: $1,830

• Deposit: $540

• Remaining balance: $1,290

 

Since this is 9 sessions, the remaining balance is split across 8 sessions:

Payment schedule:

• Session 1 (deposit): $540

• Sessions 2–9: $161.25 per session

 

Submission of this form does not guarantee placement. To secure guaranteed placement in any Skin Package Program, a deposit must be completed within 48 hours. A payment invoice will be sent after this form is submitted. Enrollment is only secured once the deposit is received. If split payments are requested, payment timelines will be discussed.
