1. Investigation of Safety Performance History Pristine Transport Inc. is required by federal law to investigate your safety performance history for the three years immediately preceding the date of this application. This inquiry will include:
Employment Verification: Confirmation of your previous driver identification and employment dates.
Accident Record: Details of any accidents as defined in 49 CFR 390.5T.
Drug/Alcohol History: Verification of any violations of alcohol and controlled substances prohibitions under 49 CFR 382 or 49 CFR 40.
2. Your Right to Review and Contest Information Federal regulations grant you specific rights regarding the information provided by previous employers:
The Right to Review: You have the right to review the safety performance information provided by your previous employers.
The Right to Correct Errors: You have the right to have errors in the information corrected by the previous employer and for that employer to re-send the corrected information to Pristine Transport Inc.
The Right to Rebut: You have the right to have a rebuttal statement attached to the alleged erroneous information if you and the previous employer cannot agree on the accuracy of the record.
3. Motor Vehicle Record (MVR) & Annual Review
Initial Pull: We are required to obtain your MVR from every State where you held a license or permit in the past 3 years. This must be placed in your file within 30 days of employment.
Annual Requirement: Pristine Transport Inc. must obtain an updated MVR for you at least once every 12 months to review your safe driving record and confirm no disqualifying offenses have occurred.
4. Drug and Alcohol Testing Disclosure
Pre-Employment Query: You must disclose if you have tested positive, or refused to test, on any pre-employment drug or alcohol test where you applied for, but did not obtain, safety-sensitive transportation work.
Mandatory Refusal: If you refuse to allow Pristine Transport Inc. to make an inquiry into your previous drug and alcohol testing history, we are strictly prohibited by federal law from permitting you to operate a commercial motor vehicle (CMV)
5. Medical & Physical Fitness Disclosure
Medical Certificate: You must be issued a Medical Examiner’s Certificate verifying you meet the physical requirements of 49 CFR 391.41.
National Registry: We are required to verify and document that your Medical Examiner is listed on the National Registry of Certified Medical Examiners.
