By signing this form I understand that I will be issued an entry code that is not to be shared. I understand that I must clean up after my group and leave the office clean and tidy (this includes - taking out the garbage, sweeping/vacuuming and wiping down tables. I understand that if it is not done my group will be charged a $75 cleaning fee and we could loose our privilege of using the Chamber Office.*

Once your form is submitted we will check the availablity of the office for you and send an email. An invoice for the deposit will also be sent, door codes will not be issued until the deposit payment is made.
