I hereby grant permission for the named on this form to participate in the activities of the Forest Hill T-Ball Association and agree to insure my child against any loss, injury or damages resulting from these activities. I agree to indemnify and save harmless the Forest Hill T-Ball association, its executive members, umpires and volunteers from all losses, claims, demands and lawsuits occasioned thereby. Further, I hereby authorize the coach to secure medical advice and services as deemed necessary for the health and safety of my child. I agree to accept financial responsibilities in excess of the benefits allowed by Provincial insurance. By signing this form all parties with legal rights to the above child are also bound by this waiver.

The season starts Saturday, May 2nd. Coaches will be contacted in early April, and parents will receive a parent package via email in mid-April. Parents will also be contacted by their child’s coach before the season begins.

Prices include a hat, shirt, trophy, individual and team photos, and a free end-of-season Family Fun Day.

As a not-for-profit, volunteer-run association, we rely on volunteer coaches. Please consider co-coaching your child’s team; there is a box to indicate interest in the parent information section. Thank you!
