1. Program Participation
For my child to participate in the Mozart Preschool Program at the following location
- Bellevue Academy of Arts & Music, 12356 Northup Way, Suite 103, Bellevue WA 98005, and its buildings and all businesses associated with the North Park Business Center at 12328–12372 Northup Way, Bellevue WA 98005
I hereby waive, release, and discharge any and all claims for damages for personal injury, and property damages or which may hereafter occur to me and my child as a result of participation in said programs.
This release is intended to discharge in advance the venue of Bellevue Academy of Arts & Music (BAAM), teachers, employees, family members, volunteers, and peers (students) from liability, even if such liability arises from perceived negligence. It is understood that some activities involve an element of risk, and knowing those risks, I hereby assume them. This waiver, release, and assumption of risk is binding on my heirs and assignees.
2. Conduct and Withdrawal
It is understood that my child must abide by the rules and codes of conduct at all program venues. If the child exhibits excessive aggression or disruptive behavior, program staff reserve the right to dismiss the child without refund.
Important Disclosure:
Students may be withdrawn from the program for any of the following:
- Unresponsiveness to instruction: Demonstrating one or fewer active responses out of five continuous prompts by the end of the second class, while over 90% of other students follow prompts. Examples include “Please bring your book and sit” or “Open the book to page 15.”
- Disruptive behavior: Significantly impeding class (pauses longer than 30 seconds per occurrence, up to three times per class, due to verbal, visual, or physical actions, such as screaming, pounding instruments, running, talking over the instructor, throwing items, or repeated behaviors). Parents of students with learning disabilities should consult staff to develop the best approach.
- Inability to follow instructions or performing the opposite action when prompts are imperative to the curriculum.
Program staff reserve the right to terminate enrollment for misconduct, including but not limited to:
- Physical contact or abuse toward others
- Verbal abuse or use of foul language
- Threats or intimidation
- Tampering with or damaging another’s property
- Unsanitary behavior
- Use, sharing, or coercing others to use inappropriate substances
All incidents will be documented in a detailed case report.
3. Medical Consent
If the child is a minor, the undersigned parent or guardian authorizes Bellevue Academy of Arts & Music (BAAM) to provide emergency medical treatment as deemed necessary while the child is participating in program activities at the listed venue.
The undersigned parent or guardian also authorizes any medical and surgical treatment, X-rays, laboratory tests, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics for the child. This authorization applies only in the event that neither parent nor guardian can be reached in an emergency.
4. Consent to Share Work as Data
By participating in BAAM programs, which include MathPiano®, PianoTowers™, and other research programs, I agree that all work created as part of participation—including recordings, compositions, assessments, and other materials—may be collected and used by BAAM for educational, research, and program development purposes.
- Collected data may be analyzed to:
- Assess student progress
- Improve teaching methodologies
- Develop new learning tools
BAAM may use aggregated, anonymized data for research studies, presentations, publications, and other academic or educational activities. Personal identifying information, including full name, address, or other sensitive details, will not be shared. Limited information, such as first name, age, and assessment results, may be publicly disclosed solely to demonstrate program outcomes and student achievements.
5. Use and Return of Materials
For students enrolled in BAAM programs, all MathPiano® and PianoTowers™ materials are copyrighted original works and must remain at BAAM. These materials, whether physical or digital, may not be taken home, retained, copied, or distributed.
Upon termination of enrollment, all materials must remain with BAAM. Unauthorized possession or duplication of MathPiano® or PianoTowers™ materials may result in legal action or other remedies deemed necessary by BAAM.
6. Consent for Communication
By participating in the BAAM programs, I consent the academy to contact me via email and/or text message for enquiry responses, new intake updates, class availability, registration openings, academy news, events and relevant promotions. I may withdraw this consent at any time by contacting the academy administration in writing.
7. Consent for Photo and Media
By participating in the BAAM programs, I consent the academy for my child’s photos or videos taken during school activities to be used in school publications, displays, website, or social media for educational or promotional purposes. I may withdraw this consent at any time by contacting the academy administration in writing.
8. Calender, Closures, and Missed Classes
The Mozart Preschool Program operates from September 2026 through June 2027.
Tuition remains unchanged during scheduled school closures, and no fee adjustments will be made for these dates.
In the event of severe weather or emergency conditions, class cancellations will follow announcements made by the Bellevue School District. Families are responsible for monitoring the district’s official communications for the most up-to-date information.
To ensure the health and safety of all students, children exhibiting symptoms of illness—including fever, vomiting, or other contagious conditions—must remain at home. If such symptoms arise during class, the school reserves the right to request immediate pick-up.
During weeks that include statutory holidays (Veterans Day, Martin Luther King Jr. Day, and Memorial Day), families may attend an alternative class within the same week or participate in a masterclass session offered exclusively to students of Bellevue Academy of Arts & Music.
Make-up classes for other absences are not guaranteed; however, alternative attendance may be offered at the teacher’s discretion, subject to availability.
9. Termination
Participation in Mozart Preschool Program is not subject to a fixed-term contractual commitment. However, a minimum of four (4) weeks’ written notice is required should you wish to withdraw your child for any reason. Tuition fees remain payable through the end of the notice period.
