  • This form must be filled out completely prior to booking an appointment with Jocelyn Mahayag at Pensacola Permanent Makeup.
    We will reach out to you directly for booking as soon as we have had the opportunity to review your submission.

    This form will require a picture of your brows in their current condition to be uploaded in order for it to be submitted. We suggest that you have that picture ready to go before moving forward. One clear quality photo with no makeup on the brows, no filters added, and preferably in natural lighting.
    Please be sure to read through our FAQ's page on the website for pre and post care instructions. JBean Tattoo wants you to have all of the necessary information for a successful and stress-free appointment.

  • Booking Fee & Missed Appointment Policy

    All policies are firm. Please read in full before scheduling your appointment.
  • GUESTS:to minimize distractions and due to safety and liability concerns, there will be no guests allowed in the procedure area. It’s recommended to come alone to your appointment or prepare for guests to wait in the seating area only. No unaccompanied children please

     

    BOOKING FEES:All booking fees are nonrefundable no exceptions. Booking fees are non transferable between clients. If the client needs to reschedule their appointment at least 48 hour notice is required to do so.

    In the event of a last minute cancellation (less than 48 hour notice given) you will forfeit your booking fee and a new booking fee must be placed in order to be rescheduled.

     

    - If the client cancels the same day as the appointment
    - If the client is late to the scheduled appointment by 15 minutes or more.
    - if the client is a ‘no call-no show’

    The appointment will be cancelled and the booking fee will be forfeited.

    The client will be responsible for paying a new booking fee before a new appointment will be scheduled.

    Booking fees will be paid at the time of booking after your appointment date and time has been selected with Jocelyn.

    Booking fees will be paid by the payment apps below. When sending booking fees please state "PMU Booking Fee/ Your Last Name"

    The booking fee does go toward the total cost of the procedure.

  • You must disclose if you have ANY previous permanent makeup for approval, no matter how long ago it was done or how faded it may appear to you.

  • We can not perform any permanent eyebrow procedure on anyone who has had any surgery done to the face/eye area within a minimum of 6 weeks. This includes but is not limited to face lift, brow lift, blepharoplasty, cataract, lasik, nose, hairline, and cheek.

  • We recommend waiting longer than 6 weeks after your surgery as we place quite a bit of pressure on the face while working and you will still be very sensitive. The skin must be stretched tightly to properly implant pigment. You will be laying down flat underneath very bright lights the entire appointment, so please keep any eye sensitivities in mind while booking. This includes your touch up appointment. Being able to lie down flat is a requirement for being an eligible candidate for any PMU procedure.

  • Please upload one full face photo. Make sure the photo is:

    • Forward facing, makeup-free, in natural, indirect lighting and contains no filters. (Sitting in your car provides the best indirect light).
    • Please wipe the brows clean with makeup remover before taking a photo.
    • Please do not send a cropped photo of just your eyebrows. We do need a full face photo with your facial muscles relaxed.


    If we're unable to assess the brows in the photo submitted, you may be asked to resend or stop by the studio.
