WAIVER & RELEASE OF LIABILITY
I, the undersigned parent or legal guardian, acknowledge and agree to the following:
1. I understand that participation in activities at Love To Play Haven involves inherent risks, including, but not limited to, falls, collisions, equipment misuse, and exposure to other children.
2. I voluntarily assume full responsibility for any risk of injury, illness, or loss, including those arising from the actions or inactions of staff, other participants, or environmental conditions.
3. I agree to supervise my child(ren) at all times during Open Play and will ensure they follow all posted rules, policies, and staff instructions.
4. I affirm that my child(ren) is/are in good health and able to safely participate in activities offered at this facility.
5. I acknowledge and agree to the session fee of $3.00 per child, per hour for participation in Open Play or Child Watch services. I understand that payment is required at check-in at the Rush County Recreation Center front desk. Parents and caregivers are admitted at no charge.
6. I agree to release, waive, and discharge the Love Community Center, Rush County Recreation Center, its employees, volunteers, affiliates, and partners from any and all liability, claims, or demands arising from participation in Open Play activities, except in cases of gross negligence or willful misconduct.
7. I authorize Love Community Center staff to provide basic first aid if needed and to contact emergency services on my behalf in the event of a medical emergency.
8. I grant permission for my child(ren) to be photographed or video recorded during participation for promotional or documentation purposes, unless I opt out in writing at the time of sign-in.
By signing below, I acknowledge that I have read, understand, and agree to the terms outlined above.
