    By submitting your story through this form, you are giving WTHellth?!, and its parent organization AiArthritis, permission to use your submission in communication pieces like our website or social media, or in our work to identify issues facing our community.

    Why do you need my location?
    Patient stories are essential to help create change that will improve our lives. This is especially true when it comes to laws, regulations, and healthcare system changes. Your story can show decision makers in your district, state, or province why change is needed. We will only share your name and/or location with legislators if you give us permission to do so.

  • Video recording only works in Chromium-based browsers (like Google Chrome, Microsoft Edge, or Brave). If you’re using Safari or Firefox, please switch to one of these browsers to record your video, or choose the “Record audio only” option above.

     

    How to Record and Submit Your Video

    Step 1 – Allow Camera & Microphone Access
    Click the "Record Video" button above. Your browser will ask for permission to use your camera and microphone - click "Allow" when prompted. If you accidentally click "Block," refresh the page and try again.

    Step 2 – Set Up Your Recording
    Under Video Settings, select "Camera Only," then click "Start Recording." A short countdown will appear - use this moment to get ready.

    Step 3 – Record Your Video
    Speak clearly and look directly at your camera. Take your time, but please note there is a 5-minute limit. When you are finished, click the "Stop" button - the red box icon located at the bottom of your browser window, next to the video preview.

    Step 4 – Review Your Recording
    Watch your recording back to make sure you're happy with it. If you're not satisfied, click the back arrow icon and select "Resume" to continue where you left off, "Restart Recording" to start over, or click the trash bin icon to delete it and begin fresh.

    Step 5 – Insert Your Video
    Once you're satisfied, click "Insert video" to attach your recording to the form.

    Step 6 – Submit the Form
    Click "Next" and complete any remaining fields, then click "Submit" at the bottom of the form. Please do not close the page until you see the confirmation message.

  • Recording your voice: Click the "Record" button to start recording, then speak clearly into your device’s microphone. When you’re finished, click the stop button and then click Next.
