I understand that the event will provide food and beverages to the partisipating youth; therefor I am responsible for reporting any food alergies, and/or any necessary medical information to the Student Arts Foundation's staff on or before the event.

A $5 registration fee per child is required to reserve a spot for Arts Day. This fee is fully refundable upon attendance at the event, or families may choose to donate the fee to support Student Arts Foundation’s youth arts programs.
