3. TWO LETTERS OF RECOMMENDATION, OR EVIDENCE OF WORK EXPERIENCE.

1. ACADEMIC - Submitted by a teacher, counselor, or administrator AND
2. PERSONAL - Submitted by an individual who can objectively assess your abilities and character (non-family member). OR
3. WORK EXPERIENCE – Submitted by an employer orsupervisor (non-family member)

4. COMMUNITY SERVICE CERTIFICATION FORM

5. PERSONAL SIGNATURE FORM - Please ensure that all parties have signed the applicable documents. Parent signature is required if applicant is under 18 years old on the date of application submission.

6. FREE APPLICATION FOR FEDERAL STUDENT AID (FAFSA) – Please upload a PDF copy of your FAFSA Submission Summary/Student Aid Index (SAI). 

Late or incomplete applications will not be accepted, no exceptions.

All scholarship finalists will be required to submit verification of enrollment in an educational entity, community college, trade school or four-year college/university enrollment by way of registration receipt prior to disbursement of scholarship funds.
