- Full Farm 578's directly from your county FSA office
- Full Farm APH direclty from your Insurance Agent
- Copy of your most recent transcript from the high school/college you attended
I understand Moore County Gin may request additional information from the applicant or the FSA office. I agree that if this application is accepted and I receive the scholarship award, I will be bound by the terms and conditions of the award.
If I am selected for this scholarship, I understand that the award amount will be paid directly to the college or university on my behalf. I understand that the scholarship funds will only be applied to offset financial obligations that I have incurred for tuition, room and board, and other educational expenses. I further understand that I am responsible for any tax liability incurred because of this award.
I certify that the statements that I have provided on this application are true and correct and are given for obtaining a Moore County Gin scholarship. I authorize Moore County Gin to verify the statements contained here and I understand that all personal information contained on this application will be held in confidence by the Moore County Gin office personnel. I also agree that if awarded a scholarship, I hereby give my consent to Moore County Gin to post my picture and notice of award on their social media pages and for future advertising.
