Phi Upsilon Omicron 2026 Conclave Registration Form

Please use this form to register for Phi U Conclave 2026 which will be held in Lubbock, Texas, Thursday, October 1st - Saturday, October 3rd. You will have the option to preview, print, and then submit the form once completed. If you wish to pay by check, please select this payment type and follow the directions per the form below. Regardless of payment type, you will still electronically submit your registration form. If you wish to pay by credit card, a $15.00 non-refundable convenience fee will be added to your total. A green check mark will appear once your registration has been successfully submitted. Registration Information:

• Registration Deadline: Friday, September 4, 2026 (If postmarked/submitted after this date, add $50 to registration fee.)
• One registration form is required for each Conclave attendee.
• Please check with your chapter to confirm delegate information prior to registering.
• No refunds after September 11, 2026 (Subject to a $25 cancellation fee.)
• Make checks payable to: Phi Upsilon Omicron Omega Chapter (Include attendee name(s) and Conclave 2026 on memo line of check.)

• Hotel reservation deadline to receive group rate is September 2, 2026 (based on availability). Check hotel policy regarding cancellations.

Registration Notes:

• Collegiate Chapters:
  Each collegiate chapter is entitled to ONE complimentary registration for its collegiate delegate, provided by the Phi Upsilon Omicron National Office.
  • The collegiate delegate must still complete and submit the registration form electronically.
  • No registration fee or check is required for the collegiate delegate.
• Alumni Chapters:
  Each alumni chapter may register ONE alumni delegate at a $100 reduced registration rate.
  • The alumni delegate must complete and submit the registration form electronically.
  • A check for the reduced registation fee as shown must be included with the completed form.
• Advisors:
  Advisors should register as alumni.
• All Other Attendees: 
  All other participants must submit full payment with their registration unless other special arrangements have been made.
• Payment by Check: 
  If paying by check, please include a copy of your completed registration form and mail it to Dr. Kyle Roberson.

By submitting this registration form, I hereby grant Phi Upsilon Omicron, Inc. the irrevocable and unrestricted right to publish photographs of me, or in which I may be included for editorial purposes, advertising, or any other purpose and in any other manner or medium, and to alter without restriction unless otherwise notified.  I hereby release the photographer and his or her legal representatives from all claims and liability relating to said photographs.  Please send notification to national@phiu.org if you do not wish to have photographs of you published.

Hold Harmless Participation Clause:

Phi Upsilon Omicron is committed to providing a safe and healthy conference atmosphere for our in-person attendees in partnership with the hotel and event space. The latest safety guidelines and protocols will be followed on property.

By registering for this event, you are acknowledging that an inherent risk of exposure to infectious, communicable diseases exists in any public space where people are present, even with health and safety measures in place. By attending the event, you voluntarily assume all risks related to your attendance and participation in the event, including exposure to infectious, communicable diseases and agree to release and hold harmless Phi Upsilon Omicron and its members, conference attendees, staff, and vendors from any and all claims, demands, and causes of action arising out of or relating to your attendance or participation in the event, including but not limited to release from any liability related to any illness, injury, or death resulting from your attendance at the event. You further agree that, by registering, if the event is canceled or postponed, Phi Upsilon Omicron is not liable for any hotel and/or travel expenses that you may have incurred in connection with the event.
