Heartworm Clinic - FUNDRAISER
Our heartworm clinic is open to all dogs and all owners.

This will be by appointment only and prepayment is required.

Dr. Gzybowski, DVM will be administering the heartworm test and dispensing the heartworm medication.

This fundraiser will be held at:

Valley Creek Road Animal Hospital

This will be by appointment only and prepayment is required.

We will start at Noon and will take 3 dogs every 15 minutes.

We are offering, 2 types of Medication:
- Interceptor Plus that only treats Heartworm. The pack has 6 chews for 6 months of treatment.
- Credelio Quattro that treats Heartworm, Flea and Tick. The pack is for 1 chewable to treat 1 month.
We will send you an invoice prior to the date via PayPal.

Payment will need to be made by February 20th.

What if I have 3 dogs?
- Fill out a form for each dog
- Select a time slot for each dog
- You will receive one bill that includes all dogs
What if I want the vet to look at other issues on my dog?
- This appointment is for Heartworm blood draw only.
Will I get records for my Vet?
- Yes. You will get results to give to your personal vet.
What happens if my dog test's positive and I had already purchased meds?
- You can still get your preventative meds but you will need to work with your personal vet on a treatment plan and there is a possibility for more testing.
Can I have my Meds mailed to me?
- No, you will need to pick them up
