Waiver of Liability & Medical Consent for New Song Church for Adults (18 and over)

I release New Song Church and any other parties acting on behalf of the church from liability in the event of an accident. I understand that New Song Church seeks to meet all CDC guidelines and provide safe and healthy environments for all students, volunteers, and staff. I acknowledge that participation in United Retreat may involve risks to the participant (and to the participant’s parents, guardians, and family members), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, and personal injury. In consideration for the opportunity to participate in United Retreat, the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the event. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during United Retreat or during transportation to and from the event, as well as for any medical treatment rendered to the participant that is authorized by the church staff or volunteers. By signing below, I agree to the above terms and conditions for my participation of United Retreat. 

For Minors (17 and under) parent or legal guardian must read & sign:

I acknowledge that participation in United Retreat may involve risks to the participant (and to the participant’s parents, guardians, and family members), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, and personal injury. As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above named minor participant. I agree on behalf of myself, my child named herein, or our heirs, successors and assigns, to hold harmless and defend New Song Church, its officers, directors and agents, chaperones, or representatives associated with the event, from any and all actions, claims, demands, damages, costs, expenses and all consequential damage arising from or in connection with my child attending the event or in connection with any illness or injury or cost of medical treatment in connection therewith, and I agree to compensate the organization, its officers, directors and agents, and New Song Church, chaperones, or representatives associated with the event forreasonable attorney’s fees and expenses arising therewith. Activities that your child may be participating will involve activities such as worship, small groups, campfire, dodgeball, field games, and/or swimming with adult supersvision (NSK & Dream). 

Medical Matters:
I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child.

Emergency Medical Treatment:
In the event of an emergency, I hereby give permission to transport my child to a hospital for emergency medical or surgical treatment. I wish to be advised prior to any further treatment by the hospital or doctor. In the event of an emergency, New Song Church will attempt to contact the parent and or the emergency contact.
