Thank you for your interest in Studio J’adore and booking an appointment with me!

My books are now open which means I am accepting applications for all permanent makeup services such as brows, eyeliner, and lip color, as well as tattoo appointments!

In order for me to give you the best experience and service possible, please fill out the following form as accurately and in as much detail as possible. If you are looking to book for specific dates or days/times you want to come in- please make sure to specify that on the form. Once I have received enough applications, I will review all of them and contact you in the following 1-2 weeks to hopefully schedule an appointment!

This booking period is for March 6th - June 5th. If you are wanting to book outside this time frame, please wait for a future opening.

If I do not choose your tattoo/permanent makeup service during this round of booking, this does not mean I dont want to work with you. It is strictly based on what is the best project for me at this time and what I have availability for. Terms and Conditions: Please read carefully before submitting:
- I understand and agree to the pricing listed on the previous page, as well as acknowledging that permanent makeup and tattoos are permanent changes to the body and all the information you provided is accurate to your knowledge.
- Once I reach out to book, there is a $50-$100 deposit required to secure the appointment. If it is not paid within 48 hours of the invoice, your appointment could be forfeited and given to the next customer in line. A new spot will not be guaranteed.
- I need 48 hours notice if you need to reschedule. If it is less than 48 hours notice, the deposit can be forfeited depending on the reason to reschedule. There is no guarantee I will be able to reschedule you till the next booking period, in which you would need to reapply for the service.
- No-showing appointments result in forfeiture of the deposit as well as being banned from receiving any more future services from me.
- Even if it's super last minute, message me if you cannot come.
